Objective:Based on the theory of dynamic and static combination,compare the clinical efficacy of the operation plan between Lumbar Discectomy combined with WavefleX system and Simple Lumbar Discectomy,and explore the influence of the two plans on the stress,stability and mobility after lumbar surgery,so as to provide basis for clinical selection and application.Methods:to collect 20 cases of the patients who underwent the operation of Lumbar Discectomy combined with WavefleX system and 20 cases of Simple Lumbar Discectomy.To record the operation time,bleeding volume.VAS and ODI were recorded before operation,1 month after and at the end of the follow-up.Obtain the imaging indexes(DSH,HIF and ROM)before operation and at the end of the follow-up;The finite element models of the two operation plans were constructed to obtain the stress of the disc and articular process cartilage of the responsible segment(L4/5)and the upper segment(L3/4)under five kinds of loads,and output the stress nephogram of the internal fixation to compare the biomechanical characteristics of the two models.Results:The operation time and bleeding volume of Elastic Fixation group were longer than Lumbar Discectomy group.VAS and ODI were similar in the two groups one month after operation,but Elastic Fixation group was better than Lumbar Discectomy group at the end of the follow-up.The DSH and HIF of Elastic Fixation group were larger than Lumbar Discectomy group,and ROM was smaller than Lumbar Discectomy group.Under the five loads,the disc stress,articular cartilage stress and disc shape variables of the responsible segment of Elastic Fixation model were smaller than Lumbar Discectomy model,the stress of the upper segment disc was close,and the elastic structure of WavefleX system had obvious stress concentration.Conclusion:WavefleX Elastic Fixation plan can achieve better long-term clinical efficacy,maintain the stability of fixed segment,slow down the degeneration of disc and articular process,and provide a certain degree of activity.However,compared with the Simple Lumbar Discectomy plan,its operation damage is greater and there is a risk of internal fixation fracture.
