Objective:To analyze the effect of lysophosphatidic acid(LPA)on different lesions of coronary heart disease and the prognosis of recently,and hope to achieve the purpose of predicting the degree of coronary artery stenosis in patients with coronary heart disease and assessing the prognosis of patients.Provide basis for further research and standard treatment of patientsMethods:In this study,from December 2018 to December 2019,102 patients who were hospitalized in the Department of Cardiology of our hospital who underwent coronary angiography were selected as the research subjects.According to the coronary angiography results,the patients were divided into Normal group(n=12),single-vessel disease group(n=20),double-vessel disease group(n=29)and multi-vessel disease group(n=41).According to the statistics of coronary artery stenosis and Gensini score in patients with coronary heart disease,compare the effects of LPA expression level and LPA expression level of patients with different coronary artery diseased branches,different coronary artery stenosis degrees and different types of coronary heart disease(acute myocardial infarction and non acute myocardial infarction)and effect of LPA on the severity of coronary artery lesions in patients with coronary heart disease.At the same time,Spearson’s correlation was used to analyze the correlation between LPA expression and Gensini score,as well as the correlation between LPA expression and left ventricular ejection fraction(LVEF).Using binary logistic regression analysis,at the same time by drawing the artificial receptor curve(ROC),and then using the area under the curve(AUC)to analyze the LPA expression level,so as to determine the possibility of MACE in patients with coronary heart disease.Use the analysis conclusions to evaluate the patient’s prognosis.Results:There was no significant difference in gender and age of patients in each group with different coronary artery disease counts(P>0.05).Among patients with different coronary artery disease counts,the proportions of patients with smoking history,drinking history,hypertension and diabetes were higher than those in the normal group(P<0.05)).The BMI and LDL-C of patients with different coronary artery diseased branches were significantly higher than the normal group(P<0.05),and the HDL-C was significantly lower than normal In the group(P<0.05).There was no significant difference in the general data of patients with coronary heart disease with different diseased branches(P>0.05).The independent variables with statistically significant differences between the case group and the control group include:gender,LPA,HDL-C(P<0.05).There was significant difference in the degree of coronary artery stenosis in patients with different coronary artery diseased branches(P<0.05).The Gensini scores of patients with different coronary artery lesions are different(P<0.05).The Gensini scores of patients in the multivessel disease group are significantly higher than those of the single-vessel disease group and the double-vessel disease group(P<0.05),and the double-vessel branch disease group was significantly higher than the single branch disease group(P<0.05).The LPA expression levels of patients in each group were different(P<0.05).The LPA expression of patients with coronary artery disease was significantly higher than that of the normal group(P<0.05),and The expression of LPA in the multivessel disease group was significantly higher than that of the single-vessel disease group and the double-vessel disease group(P<0.05),and the double-vessel disease group was significantly higher than the single-vessel disease group(P<0.05),and the acute myocardial infarction group higher than the non acute myocardial infarction group(P<0.05).The results of correlation analysis showed that there was a significant positive correlation between LPA and Gensini score(r=0.839,P<0.001).Correlation analysis showed that there was a certain negative correlation between LPA and LVEF score(r=-0.592,P<0.001).After a 6-month follow-up of patients,it was found that LPA expression had a better effect on the occurrence of MACE in patients with coronary heart disease.Binary Logistic regression analysis(P<0.05)and AUC assessed by ROC curve were0.765(P<0.05).Conclusion:There is a significant correlation between LPA and the Gensini score and LVEF of patients with coronary heart disease.The incidence of MACE is different in patients with different LPA expression levels.The expression level of LPA can effectively predict the occurrence of MACE in patients with coronary heart disease,so as to judge whether the prognosis of patients with coronary heart disease is good.At the same time,it is suggested that LPA is closely related to the degree of coronary artery stenosis.Therefore,the expression level of LPA can also be used as a biological indicator to predict the degree of coronary artery stenosis,which is beneficial to the development of clinical treatment. |
