Clinical Study On The Intervention Of Acute Coronary Syndrome With Qi Deficiency Blood Stasis Phlegm Turbidity Syndrome From The Perspective Of Internal Carbuncle Of Traditional Chinese Medicine

Objective:To observe the clinical efficacy and safety of TCM detoxifying and dredging collaterals for acute coronary syndromes through clinical studies,and to analyze the particularity and universality of TCM prescriptions for the treatment of acute coronary syndromes,so as to open up new ideas for the treatment of acute coronary syndromes with traditional Chinese medicine.Methods:In this study,120 ACS patients were divided into three groups for clinical observation.Clinical observation was conducted for 4 weeks.Control group A was given standardized treatment of Western medicine(aspirin,clovigrel,atorvastatin,ramipril and low molecular weight heparin,etc.),control group B was given standardized treatment of Western medicine plus the diagnosis and treatment standards of the Department of Heart Disease of the Affiliated Hospital of Changchun University of Chinese Medicine,and the treatment group was given Jiedu Tongluo prescription on the basis of control group B.Relevant clinical cases were collected,data were collected and analyzed,and various indicators of patients in the three groups before and after treatment were compared according to the principle of trial design,including Seattle Angina Scale score,TCM symptom score,improvement of blood lipid level,hs-CRP changes and abnormal liver and kidney function before and after treatment.Accurate statistics of relevant data,and draw conclusions by comparison.Results:(1)Comparison of the curative effect of angina pectoris: there were significant differences between the treatment group and control group A and B respectively(P<0.05),indicating that the efficacy of the traditional Chinese medicine group of detoxification and dredging collateral was better than that of the two control groups,and the method of detoxification and dredging collateral was related to modern pathological mechanism of ACS.(2)Seattle scale score comparison: pairwise comparison among the three groups showed significant differences(P<0.05),indicating that the improvement of angina pectoris was the most obvious in the group of detoxifying collaterals,followed by the group of Chinese medicine with diagnosis and treatment plan.(3)Comparison of TCM symptom scores: Pairwise comparison among the three groups showed significant differences(P<0.05).The detoxifying and dredging collaterals group improved TCM symptoms most obviously,and detoxifying and dredging collaterals group helped to improve TCM symptoms of ACS patients.Considering that the TCM pathogenesis of ACS was closely related to "heat toxicity and blood stasis".(4)Comparison of blood lipid levels: there were significant differences between the two groups(P<0.01).The Jiedu Tongluo prescription had the most obvious effect of regulating blood lipid,and Jiedu Tongluo prescription had a good function of regulating lipid metabolism.(5)Comparison of hs-CRP concentration: there were significant differences between the treatment group and the two control groups(P<0.01).The reduction of CRP concentration in the Jiedu Tongluo TCM group was the most obvious.This prescription could significantly reduce the inflammatory response and alleviate or block the inflammatory response process of ACS.(6)Safety evaluation: Before and after treatment,no major adverse reactions were observed in routine blood,urine,stool,liver and renal function examinations of 120 patients in this study.Only 1 patient in control group A showed mild hemoglobin decrease during treatment,and no special treatment was given.No other obvious abnormalities were observed.Conclusion:(1)The method of detoxification and dredging collaterals is an effective treatment for the key TCM pathogenesis of ACS without obvious adverse reactions,and "heat toxicity and blood stasis" is the key TCM pathogenesis of ACS;(2)the detoxification of angina pectoris in patients with Jiedu Tongluo method in improving the ACS,TCM symptoms and physical and chemical indicators are significant difference compared with the two control group,western medicine standardized treatment efficacy,prompt detoxification t2dm method to further improve the treatment of ACS advantages of traditional Chinese medicine,effective for most of the patients with ACS,the clinical value of universal application.
