| This thesis was consisted of two chapters and summarized the research works during the master’s study.In the first chapter,spectrum and classification of common diseases in department of neurology of A Hospital were introduced.In the second chapter,the evidence-based evaluation of drug use in the department of neurology of the A Hospital was conducted.In addition,the research on drug safety of nervous system diseases based on evidence-based medicine is reviewedObjective(1)The spectrum of common diseases in the department of neurology of the A Hospital was counted using a retrospective survey.(2)The spectrum of common neurological internal diseases was classified according to the international classification of diseases(ICD-10).(3)The existing contents of relevant drug varieties in A Hospital were analyzed,collated and evaluated(4)Relevant evidence-based queries were made according to the common neurology disease spectrum.(5)The consistency of the disease spectrum and the drug catalog is assessed(classified by level of evidence).(6)To evaluate the consistency of the disease spectrum and current drug supply catalog of inpatients in A Hospital,and to derive the accessibility and shortage drug catalog of the basic drug catalog of neurological internal medicine specialists.Methods(1)The spectrum of common diseases in the department of neurology of the A Hospital was counted using a retrospective survey.(2)The spectrum of common neurological internal diseases was classified according to the international classification of diseases(ICD-10).(3)The existing contents of relevant drug varieties in A Hospital were analyzed,collated and evaluated.(4)Relevant evidence-based queries were made according to the common neurology disease spectrum.(5)The consistency of the disease spectrum and the drug catalog is assessed(classified by level of evidence).(6)To evaluate the consistency of the disease spectrum and the current drug supply catalog of inpatients in the A Hospital,and to derive the accessibility and shortage drug catalog of the basic drug catalog of neurological internal medicine specialists.Results(1)In the A Hospital in 2019,there were 6028 inpatients in the department of neurology.Among the top 10 diseases in the department of neurology,transient ischemic attack,cerebral infarction and migraine were the top three,accounting for 71.52% of the total number of diseases;(2)According to the disease spectrum,92 kinds of first-line drugs in neurology department were recommended.Among them,17 kinds of drugs were recommended with frequency more than 2,16 kinds of drugs were in use in neurology department,1 kind of drugs was short of recommended drugs in disease spectrum,and the consistent rate of first-line recommended drugs in neurology department was 94.1%;78 kinds of second-line drugs in neurology department were recommended based on disease spectrum.Among them,13 kinds of drugs with more than 2 recommended frequency,10 kinds of drugs in use in neurology department,and 3 kinds of drugs in shortage were recommended by disease spectrum.The coincidence rate between disease spectrum recommendation and drugs in use was 76.9%;There were 78 kinds of drugs in use in the department of neurology of the A Hospital,but 7 kinds of drugs were not recommended by the disease spectrum;(3)The neurology department of the A Hospitalis highly consistent with the recommendation of evidence-based first-line and second-line drugs.Except for some imported drugs,most of the drugs in use are in the national basic list,which basically ensures the rationality and economy of drug use in neurology department.However,due to some irrationality of clinical diagnostic criteria,clinicians do not use drugs reasonably according to the relevant evidence-based guidelines for some diseasesConclusion There are some differences between the first-line and second-line drugs used in the common diseases hospital of the A Hospital and those listed in the evidence-based basis.Most of them are used in the national basic list,which basically ensures the rationality and economy of drug use in the department of neurology.Due to the irrational diagnosis of some diseases,the rationality of drug use remains to be discussed.With the extensive development of clinical trials in various disciplines,the evidence of evidence-based medicine has been continuously improved.Under the premise of correctly using evidence-based medicine to guide clinical practice,it has laid the foundation for the writing of clinical guidelines and expert consensus,standardized clinicians,provided patients with high-quality medical services and saved medical costs,and promoted the continuous development and progress of medicine. |
