| ObjectiveTo investigate the current situation of hyperphosphatemia in patients undergoing peritoneal dialysis,to explore the non-disease influencing factors of hyperphosphatemia in patients undergoing peritoneal dialysis,which provided the basis to help the medical staff to find high risk groups,to formulate personalized intervention measures to assist clinical treatment,and to reduce the blood phosphorus level.MethodsThis study is a cross-sectional study,using a convenient sampling method,from October 2019 to October 2020,180 patients undergoing peritoneal dialysis in a Grade3 A hospital in Dalian were selected.General information questionnaire,Chronic Disease Self-Efficacy Scale(CDSES),Social Support Rating Scale(SSRS),Chronic Kidney Disease-Mineral and Bone Disorder Knowledge and Behavior(CKD-MBD-KB)were used to investigate.Data were entered in pairs,and data analysis were performed using SPSS24.0 software,statistical methods include descriptive statistics,chi-square test,t test and Logistic regression analysis.Results1.The blood phosphorus concentration of patients undergoing peritoneal dialysis was(1.97±0.57)mmol/L,58.9% patients with hyperphosphatemia.2.A total score of self-efficacy in patients undergoing peritoneal dialysis was(6.81±0.93),the scores of two dimensions from high to low were: symptom management self-efficacy(6.81 ± 1.02),common disease management self-efficacy(6.79 ±1.08).The total score of social support was(37.88 ± 6.47),and subjective support was(23.32±4.33),objective support was(8.63±1.82),utilization of support was(5.94±2.21).A total score of CKD-MBD-KB was(120.88±10.48).The average descending order of each dimension in the field of knowledge were: medication knowledge(1.11±0.49),disease knowledge(0.75±0.23),diet knowledge(0.70±0.45).In the field of behavior respectively were :treatment attitude(4.78±0.22)and treatment compliance(3.96±0.44).3.Univariate analysis showed that the patients undergoing peritoneal dialysis in hyperphosphatemia group and non-hyperphosphatemia group was statistically significant between different ages,education level and peritoneal dialysis age group(P<0.05).Self-efficacy,objective support,utilization of support,diet knowledge,medication knowledge and treatment compliance were the influencing factors of hyperphosphatemia in patients undergoing peritoneal dialysis(P<0.05).4.Logistic regression analysis showed that peritoneal dialysis age,dietary knowledge,medication knowledge,treatment compliance,and symptom management self-efficacy were independent factors influencing hyperphosphatemia in patients undergoing peritoneal dialysis(P<0.05).Conclusion1.The incidence of hyperphosphatemia in patients undergoing peritoneal dialysis was high,and blood phosphorus control was not ideal.Self-efficacy,social support and the knowledge and behavior of hyperphosphatemia in patients undergoing peritoneal dialysis needed to be improved.2.The non-disease factors of hyperphosphatemia in patients undergoing peritoneal dialysis mainly include: peritoneal dialysis age,diet knowledge,medication knowledge,treatment compliance,symptom management self-efficacy.Medical staff should consider the above aspects,find and focus on the high risk population prone to hyperphosphatemia,carry out targeted health education and individualized psychosocial cognitive intervention measures on the basis of clinical treatment,in order to effectively assist the clinical treatment plan and better reduce the blood phosphorus level. |
