Epidemiological Investigation Of Mineral And Bone Metabolism In Peritoneal Dialysis And Analysis Of Influencing Factors

Objective:To retrospectively analyze the biochemical indexes such as calcium phosphorus and iPTH in peritoneal dialysis center in Second Hospital,Shanxi Medical University in recent 5 years,analyze the causes of abnormal mineral and bone metabolism,and further improve the quality of life and prognosis of patients with peritoneal dialysis.Methods:102 patients with long-term maintenance peritoneal dialysis were selected from the Second Hospital of Shanxi Medical University from January 2014 to December2018.Learn about calcium and phosphorus metabolic disorders,and divide them into blood calcium,phosphorus and iPTH levels.The difference of clinical data between the two groups was compared between the standard group and the non-standard group,and the influencing factors of calcium and phosphorus metabolism in some areas of Shanxi Province were discussed.Results:In peritoneal dialysis patients,the ratio of male to female was 0.79 ≤ 1,the age was(51.19 ±13.89)years old,the mean serum calcium,phosphorus,iPTH and ALP levels were(2.07 ±0.26)mmol/L,(1.77 ±0.52)mmol/L,(387.82 ±40.26)pg/mL,(103.86 ±10.28)U / L,and the mean serum calcium,phosphorus,I at the last follow-up were(1.77 ±0.52)mmol/L,(1.77 ±0.52)pg/mL,(103.86 ±10.28)U / L,respectively.The levels of PTH and ALP were(2.16 ±0.27)mmol/L,(1.96 ±0.79)mmol/L,(497.79 ±39.62)pg/mL,(101.41 ±10.040)U / L,respectively.The reachingrates of serum calcium,phosphorus and iPTH were 37.25%,48.04% and 19.61%,respectively.at the last follow-up,the reaching rates of serum calcium,phosphorus and iPTH were 48.04%,34.31% and 13.73%,respectively.Analysis of factors controlling substandard of calcium and phosphorus Metabolism Indexes: serum Creatine at baseline is substandard in Blood calcium The influencing factors: the product of baseline calcium and phosphorus is the influencing factor of blood phosphorus substandard,the dialysis age is the influencing factor of blood iPTH substandard,and the dialysis age,the base line ALP is the influencing factor of ALP substandard.Conclusion:The disorder of calcium and phosphorus metabolism in peritoneal dialysis patients is prominent,the rate of blood calcium,phosphorus and iPTH is not ideal;the factors affecting calcium and phosphorus metabolism are serum creatinine,dialysis age,calcium and phosphorus product;age and diabetes are risk factors for phosphorus deficiency in peritoneal dialysis patients;peritoneal dialysis patients should be individualized treatment,strengthening diet management and health education of PD patients may be effective means to solve the disorder of calcium and phosphorus metabolism,and explore suitable local peritoneal dialysis methods.
