Development Of The Treatment And Rehabilitation Programme For Rotator Cuff Injury Based On Evidence-based Medicine And Delphi Expert Evaluation

Objective1.Based on the principle of evidence-based medicine,through literature research,this paper makes a systematic and comprehensive study on the literature related to the diagnosis and treatment of rotator cuff injury,so as to provide evidence-based medicine basis for the formation of the draft guidelines for TCM diagnosis and treatment of rotator cuff injury.2.0n the basis of previous literature research,through Delphi expert questionnaire survey,the common pathogenic factors,lifestyle intervention and related factors affecting the curative effect of rotator cuff injury were discussed,and the consensus of experts was formed.it also provides a reference basis for the further formulation of the guidelines for TCM diagnosis and treatment of rotator cuff injury.Methods1.Literature retrieval,screening,quality evaluation and evidence classificationBased on the principle of evidence-based medicine,we systematically searched the relevant literatures on the diagnosis and treatment of rotator cuff injury in China knowledge Network,Wanfang,VIP,PubMed,TheCochrane Library,EMbase and other domestic and foreign databases.The search deadline is 2020-6-31.The screening of the original literature should strictly follow the inclusion and exclusion criteria,and the corresponding quality evaluation tools should be selected for different types of literature.The documents with improved Jadad score≥3,MINORS entry≥ 13 and AMSTAR scale score≥5 were included,and the evidence was classified and recommended according to the classification standard of TCM literature,so as to provide evidence support for the formulation of the guide.2.Based on Delphi expert questionnaire surveyOn the basis of previous literature research,following the principle of Delphi method,an expert questionnaire was developed about common pathogenic factors,lifestyle intervention and related factors affecting the curative effect of rotator cuff injury.60 orthopedic experts in China were investigated by electronic questionnaire through questionnaire Star tool(www.wjx.cn).By calculating the enthusiasm of experts,the degree of concentration of expert opinions,the degree of coordination and other indicators,collect expert opinions,and add or modify relevant items according to expert opinions,finally,an expert consensus was reached on the common pathogenic factors of rotator cuff injury,lifestyle intervention and related factors affecting the curative effect of operation.ResultsUnder the guidance of the principle of evidence-based medicine,this paper makes a systematic and comprehensive study on the literature related to the diagnosis and treatment of rotator cuff injury,and strictly follows the relevant requirements of the Chinese Society of traditional Chinese Medicine,standardize the relevant contents of the process of diagnosis and treatment of rotator cuff injury from the aspects of definition,diagnosis,syndrome differentiation,treatment,functional exercise,prevention and nursing,and initially form the guidelines for diagnosis and treatment of rotator cuff injury(draft).At the same time,on the basis of previous literature research,through two rounds of Delphi expert questionnaire survey,we finally reached an expert consensus on the common pathogenic factors of rotator cuff injury,lifestyle intervention and related factors affecting the curative effect of operation.the purpose of this study is to provide a reference basis for the further formulation of the guidelines for TCM diagnosis and treatment of rotator cuff injury.ConclusionThe research result of this subject-"guidelines for TCM diagnosis and treatment of rotator cuff injury"(draft),in the formulation process,in strict accordance with the relevant requirements made by the Chinese Society of traditional Chinese Medicine,follow the principles of evidence-based medicine,fully reflect the characteristics and advantages of traditional Chinese medicine,but also reflect the latest evidence of evidence-based medicine,provide the best diagnosis and treatment advice,hoping to have a better guiding significance for clinical practice.However,due to time constraints,the draft guidelines have not been demonstrated by the expert consensus meeting and clinical consistency evaluation,and further work needs to be carried out in the follow-up.In addition,through the Delphi questionnaire survey,two rounds of expert opinions were consulted on the common pathogenic factors of rotator cuff injury,lifestyle intervention and related factors affecting the curative effect of operation,and the expert opinions were fully summarized and reached an expert consensus.it not only provides a basis for the treatment and rehabilitation of rotator cuff injury,but also provides a reference basis for the further formulation of guidelines.
