| Objective: The purpose of this study was to observe the effect of Lanqingqiang empirical prescription of Lanqingqiang prescription on TCM symptom score,liver indexes,hemodynamics,spleen size,ascites depth and the degree of esophageal and gastric varices in patients with liver cirrhosis and portal hypertension and the incidence of esophagogastric vein bleeding and hepatic encephalopathy within half a year after the end of the course of treatment.To explore the effect of Ganning prescription on improving collateral circulation in patients with hepatitis B cirrhosis and portal hypertension.Methods:According to the inclusion criteria,among the outpatients and inpatients in the Department of Gastroenterology and Department of Hepatology of Ruikang Hospital affiliated to Guangxi University of traditional Chinese Medicine,60 patients with portal hypertension due to hepatitis B cirrhosis with open collateral circulation and dialectically belonging to the syndrome of liver depression and blood stasis from December 2018 to June 2020 were randomly divided into treatment group(n= 30)and control group(n = 30).The control group was treated with propranolol hydrochloride tablets(propranolol)and the treatment group was treated with Ganning Formula.The clinical indexes such as coagulation function,liver function,liver fibrosis,portal vein hemodynamics,cardiac hemodynamics,spleen size,ascites depth and the degree of esophageal and gastric varices were observed in the two groups before and after treatment.The incidence of esophagogastric vein rupture and hepatic encephalopathy were followed up within half a year after the course of treatment.Results:(1)baseline data: there was no significant difference in age,sex,Child-Pugh score of liver function and course of disease between the treatment group and the control group.(2)TCM syndrome scores: after treatment,the TCM syndrome scores and total scores of the two groups were significantly lower than those before treatment(P < 0.05),and the decrease in the treatment group was more obvious than that in the control group(P < 0.05).The total effective rate of 30 patients in the treatment group was 90%.The total effective rate of the control group was 56.6%,and the curative effect of the treatment group was significantly better than that of the control group(P < 0.05).(3)Coagulation function: after treatment,the level of PT in the treatment group was significantly lower than that in the control group,and the level of PTA in the treatment group was significantly higher than that in the control group(P < 0.05),and the increase in the treatment group was more obvious than that in the control group(P < 0.05).(4)liver function: after treatment,the levels of TBIL,ALT and AST in the treatment group were significantly lower than those in the control group,and the level of ALB in the treatment group was significantly higher than that in the control group(P < 0.05),and the increase in the treatment group was more obvious than that in the control group(P < 0.05).(5)Indexes of hepatic fibrosis: the levels of HA,LN,PC Ⅲ,Ⅳ-C and LSM of liver fibrosis in the treatment group after treatment were significantly lower than those before treatment,and the levels of HA,LN,Ⅳ-C and LSM in the control group after treatment were significantly lower than those in the control group(P < 0.05).(6)Portal vein hemodynamics: the levels of DPV,DSV and QPV of portal vein hemodynamics in both groups after treatment were significantly lower than those before treatment,and the decrease in the treatment group was more obvious than that in the control group,and the level of SPV in the treatment group was significantly higher than that in the control group(P < 0.05).(7)Cardiac hemodynamic indexes: the levels of cardiac hemodynamics HR and CO in both groups after treatment were significantly lower than those before treatment,and the decrease of CO in the treatment group was more significant than that in the control group(P < 0.05).The level of MAP in the treatment group was significantly higher than that before treatment,and the increase in the treatment group was more obvious than that in the control group.(8)the spleen length,spleen thickness and ascites depth in the treatment group after treatment were significantly lower than those before treatment(P< 0.05),and the decrease in the treatment group was more obvious than that in the control group(P < 0.05).The depth of ascites in the control group decreased significantly after treatment(P < 0.05).(9)the degree of esophagogastric varices in the treatment group was significantly improved after treatment(P < 0.05),and the decrease in the treatment group was more obvious than that in the control group(P < 0.05).(10)the incidence of esophagogastric vein rupture bleeding and hepatic encephalopathy: among the 30 patients in the treatment group within half a year,the total incidence of esophagogastric vein rupture bleeding was 10%,and the total incidence of hepatic encephalopathy was 6%.The total incidence of esophageal and gastric vein rupture bleeding in the control group was 33%,and the total incidence of hepatic encephalopathy in the treatment group was significantly lower than that in the control group(P < 0.05).Conclusions: 1.Ganning prescription has a good clinical effect and application prospect in the treatment of hepatitis B cirrhosis and portal hypertension.It can improve the clinical efficacy indexes such as blood coagulation,liver function and liver fibrosis,reduce ascites and improve splenomegaly.And it can effectively improve the degree of liver fibrosis,portal vein and cardiac hemodynamics,and achieve the effect of reducing portal vein pressure.2.While reducing portal pressure,Ganning prescription can effectively reduce the incidence of complications related to collateral circulation opening in patients with hepatitis B cirrhosis and portal hypertension,and effectively reduce the morphology of esophageal and gastric varices in patients with hepatitis B cirrhosis and portal hypertension.and can reduce ascites,reduce the incidence of esophagogastric vein rupture bleeding and hepatic encephalopathy.Ganning prescription has the potential to improve the varicose shape of collateral circulation and delay the formation of collateral vessels. |
