Clinical Observation On Treatment Of Anterior Ischemic Optic Neuropathy Based On The Theory Of "Desolation Time Of JueYin Disease"

Objective:In recent years,the incidence of anterior non-arteriammatory ischemic optic neuropathy(NAION)in China is gradually increasing and the age of the patients is younger.Based on the onset time and syndrome characteristics of NAION,we propose that the main pathogenesis of NAION is that Qi of Yin and Yang are not connected with each other and Qi and blood cannot be fed to the eyes.Therefore,NAION belongs to the category of JueYin disease.Based on this idea,we chose Wumei Pill,the main prescription of Jue Yin disease,in order to harmonize Yin and Yang and restore the Qi and blood of the eye system,to provide new ideas and methods for TCM treatment of NAION.Methods: In this study,40 patients who met the criteria were included,20 patients in the experimental group and 20 patients in the control group.The experimental group was given Wumei pill and the control group was given traditional Chinese medicine of activating blood circulation.The changes of best corrected visual acuity(BCVA),vision field Mean Deviation(MD),Pattern Standard Deviation(PSD),glycosylated albumin(GA),whole cholesterol(CHOL)and systemic syndrome in 2 groups before and after treatment were compared and analyzed,so as to evaluate the efficacy.Results:(1)Basic situation: NAION was mostly developed at Desolation time of Jue Yin disease(68%),and was correlated with hypertension,diabetes and dyslipidemia.(2)Baseline comparison: There were no significant differences in age,BCVA,MD,PSD,GA,CHOL,TCM symptom scores between the two groups before treatment(P<0.05).(3)After 1 course of treatment,TCM symptom scores in both groups were significantly decreased(P<0.05).BCVA and MD were improved in the experimental group(P<0.05)and levels of CHOL and GA were lower than before(P<0.05).There was a statistical difference only in TCM symptom scores between the two groups(P<0.05).(4)After 1 course of treatment,the total effective rate of the experimental group was 45%,and the total effective rate of the control group was 10%,but there was no statistical difference between the two groups.(5)After 2 courses of treatment,BCVA,MD and PSD were better in both groups;GA and TCM symptom scores were lower than before(P<0.05).The CHOL level in the experimental group decreased(P<0.05).The effective indexes of the experimental group were better than those of the control group(P<0.05).(6)After 2 courses of treatment,the total effective rate of the experimental group was 70%,the total effective rate of the control group was 30%,the experimental group was significantly better than the control group(P<0.05).Conclusions: Based on the characteristics of patients’ onset and syndrome,the idea of treating NAION from Jue Yin disease can effectively improve patients’ visual,vision field and syndrome,which has good clinical application value and also provides a new idea for the treatment of this disease.
