Abstract/Summary:
Objective: Depression and anxiety are the most common associated conditions of Primary Sjogren’s syndrome(p SS),and the incidence rate is also higher. Objective to study the incidence of depression and anxiety in patients with p SS and its correlation with the general situation of patients, disease activity, somatosensory,TCM syndrome elements,so as to provide new thinking for TCM treatment of p SS.Methods:The general situation of patients,EULAR Sjogren’s syndrome disease activity index(ESSDAI),EULAR Sjogren’s syndrome patient reported index(ESSPRI)and the medical outcomes study item short from health survey were collected and analyzed by observational study,SF-36,self rating Anxiety Scale(SAS),self rating Depression Scale(SDS),FSS(FSS)and TCM syndrome elements were used to establish a database of304 PSS patients who met the inclusion criteria.SPSS24.0 was used to analyze the correlation between SAS,SDS and ESSDAI,ESSPRI,SF-36,SF-36 Correlation between FSS and TCM syndrome elements.Results:(1)Among 304 patients with p SS,115 patients were in depression state, accounting for 37.83% of the total number,of which severe depression accounted for 10.9%,89 patients were in anxiety state, accounting for 29.3% of the total number, and severe anxiety accounted for 4.9%.(2)The incidence of depression and anxiety was not correlated with gender(P >0.05),but correlated with age(P <0.05).The course of disease was only correlated with anxiety(P <0.05),but not with depression(P >0.05).(3)The depression and anxiety of p SS patients were correlated with the scores of ESSDAI,ESSPRI,SF-36 and FSS(P < 0.05),and the higher the scores of ESSPRI and FSS,the more likely the depression and anxiety of p SS patients were; The higher the SF-36 score, the lower the possibility of depression and anxiety in p SS patients.(4)In p SS patients, both yin deficiency and Jin deficiency syndrome and Qi and yin deficiency syndrome are prone to depression and anxiety, while Yin deficiency and heat toxin syndrome is prone to anxiety; The SDS score of patients with Qi Yin deficiency syndrome was significantly higher than that of patients with Yin deficiency and Jin deficiency syndrome; The SAS score of patients with Qi Yin deficiency syndrome was significantly higher than that of patients with Yin deficiency and blood stasis syndrome.(5)PSS patients with Qi deficiency, fire toxin and yin deficiency were more likely to have anxiety, while PSS patients with dryness and Qi deficiency were more likely to have depression. There was no significant difference in the occurrence of depression and anxiety among patients with Yang deficiency, blood stasis and blood deficiency syndrome elements. Conclusion: The depression and anxiety of p SS patients are related to the disease activity, quality of life and symptoms of p SS.The distribution of depression and anxiety in syndrome types and elements is also different. In clinical practice,we should give full play to the characteristic theory of preventive treatment of diseases in traditional Chinese medicine, practice the concept of integration of form and spirit in traditional Chinese medicine, evaluate the depression and anxiety of patients, intervene in advance, prevent their emotional problems, pay attention to and solve the emotional problems of PSS,and carry out corresponding treatment,so as to improve the clinical level and clinical efficacy, which is worthy of further attention, application and promotion.
