The Effect Of "Internet +" Health Coaching Program On Disease Perception And Quality Of Life In Patients With Nasopharyngeal Carcinoma Undergoing Radiochemotherapy

ObjectiveTo investigate the disease perception,self-efficacy,and quality of life in in patients with nasopharyngeal carcinoma undergoing radiochemotherapy,and to analyze their influencing factors and explore the correlation between them.To explore the impact of "Internet +" Health Coaching program in patients with nasopharyngeal carcinoma undergoing adiochemotherapy,and provide practical experience for improving the quality of life of nasopharyngeal cancer patients and innovative health education models.Methods(1)The self-made general data questionnaire,the simplified version of the Disease Perception Questionnaire(BIPQ),the General Self-Efficacy Scale(GSES)and the Quality of Life Scale(QLQ-C30)were used to collect the disease perception,self-efficacy,and quality of life in 210 nasopharyngeal carcinoma patients undergoing adiochemotherapy,to analyze the influencing factors and the correlation among the three.(2)Guided by Cognitive Behavior Theory,based on baseline surveys,literature review,and expert consultation,an intervention program of "Internet+" Health Coaching program was formed.80 cases of patients with nasopharyngeal carcinoma undergoing radiochemotherapy,were randomly assiged to intervention group and control group.Patients in the control group received conventional ways health education.Patients in the intervention group underwent 3 mouths of "Internet +" Health Coaching program to intervene.The scores of disease perception,self-efficacy and quality of life of the two groups were tested before the intervention and after the 3 mouths intervention.The IBM SPSS23.0 and Amos23.0 software were used for the statistical analyzing.Results(1)The total score was(47.10±8.06).The score for cognition was(28.33±5.29)points,the score for emotions was(13.30±2.71)points,and the score for disease cognition was(5.67±1.55).Education level,medical payment form,family economy,and TNM staging were factors that affecting disease perception.(2)The total health score was(57.82±14.01)points.From the functional area,the physical function was the best(79.60±14.14);the social function score was the lowest(55.92±14.19).From the perspective of symptom field scores,decreased appetite(49.05 ± 23.55),fatigue(36.24 ± 10.50),and insomnia(35.24±23.21)were the main symptom problems.In addition,this study found that patients’ financial difficulties was more prominent(49.68±24.88).Education level,family economy,family support were the factors that affecting the quality of life of patients with nasopharyngeal carcinoma undergoing radiochemotherapy.(3)The average self-efficacy score was(22.78±5.30),of which 120patients(57.1%)were in a low level,and 76 patients(36.2%)stayed in an intermediate level,which was at a high level There were 14 patients(6.7%).Patients with high self-efficacy levels showed more active disease perception and higher quality of life,and the difference was statistically significant(P<0.05).(4)The disease perception of patients with nasopharyngeal carcinoma undergoing radiochemotherapy was negatively correlated with the scores of the quality of life and the scores of self-efficacy(P<0.05).The quality of life of patients with nasopharyngeal carcinoma undergoing radiochemotherapy was positively correlated with the scores of self-efficacy(P<0.05).Structural equation model shows that the disease perception was an intermediate variable between self-efficacy and quality of life.(5)Pre-intervention,the difference between the two groups in general information,the scores of disease perception,general self-efficacy and quality of lifethere was not significant(P>0.05).There was no statistical significance in the control group before and after intervention(P>0.05).(6)In 3 months post-intervention,the scores of disease perception,pain,fatigue,and insomnia symptoms in the intervention group were lower than those before the intervention,and the scores of general self-efficacy,role function,emotional function,social function,cognitive function and total health were higher than those before intervention(P<0.05).Compared with the patients in the control group,the physical function,emotional function,cognitive function and total health scores of the patients in the intervention group were significantly improved,and the symptoms of pain,fatigue,and insomnia were significantly improved(P<0.05).Conclusion(1)The disease perception in patients with nasopharyngeal carcinoma undergoing radiochemotherapy was influenced by education level,medical payment form,,economic conditions,and tumor staging;the quality of life was influenced by education level,family economy and family support.(2)The negative disease perception was negatively correlated with selfefficacy and quality of life(P<0.05),and the self-efficacy was positively correlated with quality of life.The disease perception was an intermediate variable between self-efficacy and quality of life.(3)The "Internet +" Health Coaching program can improve the perception of negative diseases in patients with nasopharyngeal carcinoma undergoing radiochemotherapy,and enhance the level of self-efficacy.It has a positive effect on improving the quality of life.
