Objective:To investigate the advantages of the Pediatric Early Warning Score(PEWS)and the Pediatric Critical Illness Score(PCIS)in the severity and prognosis of pneumonia in children.Methods:To collect children diagnosed as pneumonia who were hospitalized in the Department of Pediatrics of the Affiliated Hospital of North Sichuan Medical College from February 2019 to October 2020,according to the diagnostic criteria of pneumonia "Guideline for diagnosis and treatment of community-acquired pneumonia in children"(2019 version),the patients were divided into mild pneumonia group and severe pneumonia group.According to the prognosis of the disease,they were divided into the survival group and the death group.The general data,PEWS and PCIS were compared between the two groups,and the regression analysis and ROC curve of the severity and prognosis of the disease were performed.Results:424 cases were included in this study,312 cases in mild pneumonia group and 112 cases in severe pneumonia group.There were no statistically significant difference in sex,WBC,Hb,PLT,hospitalization time and PICU time between the two groups(P>0.05),the age,GR,CRP,PCT,PEWS in severe pneumonia group were higher than those in mild pneumonia group,the LY and PCIS in severe pneumonia group were lower than those in mild pneumonia group(P<0.05);410 cases in survival group and 14 cases in death group,there were no statistically significant difference in sex and Hb between the two groups(P>0.05),but there were statistically significant difference in age,WBC,GR,LY,PLT,PCT,CRP,hospitalization time,PICU time and PEWS,PCIS(P<0.05).Logistic Regression analysis showed that PEWS and PCIS were correlated with the severity and prognosis of pneumonia(P<0.05).ROC curve showed that the best cut-off values of PEWS were 2.5 points and 4.5 points,respectively,the optimal truncation values predicted by PCIS were 87 points and 77 points in severe and death children.Conclusion:WBC,GR,LY,PCT,CRP and other inflammatory indexes have some auxiliary reference value in assessing the severity and prognosis of the disease,but they are not independent risk factors;The specificity of PEWS in the evaluation of severe pneumonia in children is high,and the cut-off value of 2.5 points is suggested;PCIS may be limited in the early assessment of severe pneumonia in children.
