Migraine Among Students Of A Medical College In Western China:A Cross-sectional Study

Objective:As one of the most common primary headaches in clinical practice,migraine posing a heavy burden on individuals and society.College students are the main force of national and social future development.Migraines are most common in young college students,influencing the academic performance and quality of daily lives of medical students worldwide.This study aims to investigate the incidence of migraine among Chinese medical college students and to explore its characteristics and typical triggers.And to explore migraine with aura(MA)and migraine without aura(MOA)caused by mental disorders and differences.Method:From July 2018 to July 2019,we preliminarily screened migraineurs in North Sichuan Medical College in Sichuan province,by using ID-Migraine through cluster sampling.College students with positive ID-Migraine results would be included in this study if they were further diagnosed with migraine by neurologists based on ICHD-3.After the ethical review,patients’ personal and headache information would be collected,and the frequency,severity,onset time,and related triggers of migraine would be measured.And They are divided into MA group and MOA group according to whether they are accompanied by aura symptoms.Hamilton Anxiety Scale(HAMA)and Hamilton Depression Scale(HAMD)were used to assess headaches and mental conditions.Results:The preliminary screening covered 8783 college students,577 students were diagnosed migraine by neurologists.The overall prevalence rate of migraine is 6.57%,5.90%in men and 6.77%in women.The prevalence rate of migraine is higher for students in the first and second grades(8.01%,8.05%),and is lower in the third and forth grades(2.87%、4.67%),and students with a family history of migraine are more likely to suffer from migraine(OR=1.509,95%CI=1.060-2.148,P=0.022<0.005).Staying up late(n=329,57.01%),stress(n=319,55.29%),catch a cold(n=313,54.25%)and sleep disorders(n=302,52.34%)are the common triggers.Among them,there are 75 patients with MA(MA group)and 65 patients with one or more mental disorders(86.67%).Among the 486 MOA patients(MOA group),there are 369(75.92%)who had one or more mental disorders(P=0.039).The duration of headache and family history of migraine in the MA group were significantly different from those in the MOA group(P<0.001 and=0.018),and HAMA(P=0.005)and HAMD(P=0.000)scores of MA group were all higher than those of MOA group.Conclusion:Migraine is common among college medical students in western China.The incidence is higher among lower grade students,female students,and students with a family history of migraine.Improving sleep quality and reducing stress may be effective in relieving migraines.The incidence of migraine complicated by mental disorder among college students in medical school was high,the incidence of MA group was higher than that of MOA group,and MA had a deeper influence on mental mood.
