Objective:To evaluate the clinical applicability of frozen section rapid immunohistochemical staining in detecting perigastric lymph node micrometastases in patients with gastric cancer. Methods:A total of 69 remaining sentinel lymph nodes of 30 patients with gastric cancer who underwent surgical treatment in our hospital from 2013 to 2017 were selected for detection of tumor metastasis by rapid immunohistochemical staining, and the previous sentinel lymph node pathological detection results of the patients were collected to compare the differences between rapid immunohistochemical staining and previous lymph node pathological detection results. Results:All the sentinel lymph nodes of gastric cancer were stained by rapid immunohistochemistry in about 30 minutes.Among the 69 sentinel lymph nodes of gastric cancer, the rapid immunohistochemistry detected a total of 16 lymph nodes with macro metastasis,3 lymph nodes with micro metastasis, and 3 lymph nodes with isolated tumor cells. There was no significant difference between the results of rapid immunohistochemistry and conventional postoperative HE staining(P=0.125).Compared with the results of conventional immunohistochemical micrometastases detection in previous patients after surgery, the two methods showed good consistency in the detection of lymph node micrometastases(Kappa=0.849).There was a statistically significant difference between the results of rapid immunohistochemistry and those of previous frozen HE staining(P=0.031).Conclusion:Rapid immunohistochemistry is an accurate and rapid method for the detection of sentinel lymph node metastasis during radical gastrectomy.
