Epidemiological Characteristics And Associated Risk Factors Of Stroke In Middle And Old-aged Residents Of Hengyang Urban Area

[Objective] Understanding the epidemiological characteristics and quality of life of stroke patients in Hengyang area,analyzing the risk factors of stroke occurrence in Hengyang area,and exploring the feasible plan of community prevention and treatment of stroke are conductive to further improving the level of prevention and control of stroke and improving the quality of life of middle-aged and elderly people.[Methods] From June 2018 to December 2019,permanent residents aged 45 and above in Dongyangdu,Yejin,Guangdong Road,Miaopu and Yumu Mountain communities in Hengyang were selected as subjects for the epidemiological investigation of stroke.A total of 8500 people were investigated by cross-sectional field investigation,and 7043 cases were effectively surveyed with effective recovery of 82.85%.The field survey included face-to-face questionnaire,blood sample collection(blood lipid,fasting blood glucose),height,weight,waist circumference,pulse and blood pressure measurement.The collected data were recorded into EXECL software for sorting,and SPSS21.0 software was used for statistical analysis.Measurement data were represented by median,enumeration data by actual frequency and percentage(%),and differences between groups were tested by chi-square test.Univariate analysis and binary Logistic regression analysis were used to calculate the relative risks and 95% confidence intervals for each risk factor.p < 0.05 was statistically significant.[Results] There were 7043 valid questionnaires.Among them,267 patients had stroke,with a prevalence of 3.79%(3790/100000)and a standardized prevalence of 2.47%(2470/100000);Ischemic stroke(88.4%)was higher than hemorrhagic stroke(11.6%);The mean age was 69.0 years old,and the maximum age was 96 years old,There was a difference in the prevalence among different age groups(p<0.05).In this survey,there were 2983 males and 4060 females.Among them,139 males suffered from stroke,with a prevalence of 4.66%,and 128 females suffered from stroke,with a prevalence of 3.15%.There was a difference in the prevalence of stroke between the sexes,with a higher prevalence among males than females(p<0.05).There were differences in prevalence among people with different educational levels(p<0.05).Male smokers and drinkers were significantly higher than female(p<0.05).The prevalence of hypertension was45.56%,and the standardized rate was 36.44%;The prevalence of hyperlipidemia was 31.30%,and the standardized rate was 25.09%;The prevalence of diabetes mellitus was 14.36%,and the standardized rate was13.40%;The prevalence of coronary heart disease was 13.20%,and the standardized rate was 8.98%.Single factor analysis found that: compared with the group of cerebral apoplexy,cerebral apoplexy group of hypertension,hyperlipidemia,coronary heart disease,smoking,age,total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C),body mass index(BMI),waist circumference is different(p<0.05).Binary Logistic regression analysis showed that age,hypertension,hyperlipidemia,coronary heart disease,fasting blood glucose,triglyceride,low density lipoprotein cholesterol,high density lipoprotein cholesterol,BMI were associated with the prevalence of stroke(p<0.05).Age,hypertension,hyperlipidemia,coronary heart disease and BMI were positively correlated with the prevalence of stroke(OR>1,p<0.05),while fasting blood glucose,triglyceride,low density lipoprotein cholesterol and high density lipoprotein cholesterol were negatively correlated(OR<1,p<0.05),it is suggested that the drug use rate and diet control of stroke patients with diabetes and/or hyperlipidemia are higher than those of the high-risk population without complications,and the blood glucose and lipid results of stroke patients tend to be normal after the intervention of risk factors.[Conclusion] 1.The prevalence of stroke in Hengyang is higher than the average of other cities in China;Ischemic apoplexy was higher than hemorrhagic apoplexy.2.With the increase of age,the prevalence of stroke increases;The morbidity rate of people with high education level was lower than that of people with low education level.3.The major risk factors associated with stroke were age,hypertension,hyperlipidemia,coronary heart disease,and BMI.4.Risk factors for stroke in patients with hyperlipidemia and/or diabetes mellitus have been effectively intervened.
