Objective:To explore the application value of neutrophil to lymphocyte ratio(NLR)in the severity and prognosis of severe pneumonia in children.Methods:According to the diagnosis criteria of severe pneumonia in the 2019 "Community Acquired Pneumonia Diagnosis and Treatment of Children",the children with severe pneumonia who were hospitalized in the Pediatric Department of the North Sichuan Medical College Hospital from January 2015 to September 2020 were collected.According to the PCIS score The severity of the disease was divided into non-critical recombination and critical recombination.According to the prognosis of the disease,it was divided into the survival group and the death group.The clinical data of the two groups of children were compared respectively,and the regression analysis and ROC curve of the severity and prognosis were performed.Results:423 cases were included in this study,328 were non-critical recombination,95 were critical recombination,368 were in the survival group,and 37 were in the death group.The age,heart rate,respiration,WBC,CRP,PCT,NLR,PLR,Cr,and BUN of the critical group were significantly higher than those of the non-critical group(P<0.05),while PaO2,PLT,and pH were significantly lower than those of the critical group(P<0.05).The death group was significantly higher in month age,heart rate,WBC,CRP,PCT,NLR,Cr,BUN than the survival group(P<0.05),while PLT,PaO2,and pH were significantly lower than the survival group(P<0.05).NLR and PCT are risk factors that affect the severity of severe pneumonia in children,and their cut-off values are 1.60 and 0.89,respectively.The sensitivity and specificity of NLR are 0.874 and 0.652,respectively,and the sensitivity and specificity of PCT are 0.341 and 0.856,respectively.NLR is a risk factor affecting the prognosis of severe pneumonia in children,and its cut-off value is 2.28.The sensitivity and specificity are 0.862 and 0.756,respectively.Conclusion:NLR is an accurate,reliable and easy-to-obtain index for evaluating the severity and prognosis of severe pneumonia in children.
