| ObjectivePort wine stain(PWS)is one of the most common congenital vascular malformations,occurring in 0.3%to 0.5%of newborns[1].It initially shows flattened,pink to red patches.It may gradually develop into hypertrophic,red to purple lesions with age[2-4].By the age of 46,2/3 of patients develop papules or nodules,or overgrowth of soft tissue,resulting in deformities,asymmetry,and occasional spontaneous bleeding[5][6].Its pathological feature is the capillaries of the dermal papilla layer and reticular layer and the dilation of venules after capillaries(usually 30-300μm in diameter).The influence of PWS on the appearance of patients may seriously hinder the normal social activities and mental health development of patients,and many patients actively seek treatment.The purpose of this study was to investigate the efficacy of Helmpofin-photodynamic contrast 585/1064nm dual-wavelength laser in the treatment of different types of port wine stains,and to compare the adverse reactions of the two treatments.Methods and results40 patients with PWS treated with 585/1064nm dual-wavelength Cynergy laser from January 2015 to July 2020 were selected as dual-wavelength laser group,and 40 patients with PWS treated with HMME-PDT from November 2017 to July 2020 were selected as PDT group.Both groups received at least two Cynergy dual-wavelength laser therapy or HMME-PDT treatment.The curative effects of the two treatments were retrospectively analyzed and judged by 3 attending physicians according to the photos before and after treatment.The total effective rate of the first,second and total treatment in the PDT group was significantly higher than that in the dual-wavelength laser group.In the dual-wavelength laser group,there was no significant difference in the total curative effect of the first,second and total treatment between the purple type and the red type of PWS,but there was no significant difference in the first,second and total curative effects between the purple type and the red type of PWS in the PDT group.After the comparison of the curative effect of purple PWS between the two groups,there was no basic cure in both groups after the first treatment and the second treatment,and there was no significant difference in the total effective rate between the two groups.The total effective rate in the PDT group was significantly higher than that in the dual-wavelength laser group,and the total effective rate in the PDT group was significantly higher than that in the dual-wavelength laser group.After the comparison of the curative effect of the two groups,the total effective rate of the PDT group was significantly higher than that of the dual-wavelength laser group after the first treatment;compared with the curative effect of the second treatment,there was no significant difference in the basic cure rate,the total markedly effective rate and the total effective rate between the two groups;compared with the total effective rate of the two times,the total effective rate of the PDT group was significantly higher than that of the dual-wavelength laser group.There was no significant difference in the incidence of adverse reactions between different types of port wine stains and different groups.ConclusionThe curative effect of the first and second treatment of HMME-PDT is better than that of Cynergy dual-wavelength laser.HMME-PDT has a good curative effect on both purple type and red type PWS patients,and the adverse reactions are rare.HMME-PDT therapy is expected to replace Cynergy dual-wavelength laser in the treatment of PWS. |
