Analysis Of Influencing Factors Of Depressive Disorders In Patients With COPD And Qualitative Study Of Real Experience

ObjectiveThe prevalence of COPD patients with depression disorder was determined.To explore the influencing factors of COPD patients with depression disorder;The psychological experience of COPD patients with depression disorder was analyzed by qualitative research methods such as participatory observation and in-depth interview.It provides individualized health guidance and quality nursing services for patients with COPD complicated with depression disorder,and provides theoretical basis for improving medical staffs cognition of patients with COPD depression disorder and their real experience of depression disorder.MethodUsing literature review,survey method to collect in May 2019-December 2019 in a certain level of first-class comprehensive hospital respiratory medicine clinic of COPD patients,divided into depression and the depression group,two groups of patients by Hamilton depression rating scale for depression(HAMD)to evaluate,using logistic regression analysis to find the cause of the disorder of depression in patients with COPD influence factors,further analysis the influence of different factors on the disorder of depression in patients with COPD.Qualitative research methods such as in-depth interviews and participatory observation were used to analyze the psychological feelings of patients with COPD depression disorder during their hospitalization.Results(1)The incidence of depression in patients with copd was 31.4%.(2)Disorder of depression in patients with COPD in single factor analysis showed that depression and the depression group in sex,fee payment,smoking history,income,educational level,course of COPD,number of hospitalized patients with COPD,self-care ability,worry about disease prognosis,hospitalization expenses,factors such as family relations between differences,statistically significant difference(P<0.05);Further multivariate logistic regression analysis showed that gender,self-care ability,fear of disease and hospitalization cost were the influencing factors for COPD patients with depressive disorder.(3)Through qualitative research methods such as participatory observation and in-depth interview,psychological feelings of patients with COPD complicated with depression disorder were extracted:(1)experiencing complex emotions;(2)excessive psychological pressure,worry about the prognosis of the disease,worry about the high cost;(3)lack of emotional support.Conclusions(1)The incidence of COPD patients with depression disorder is high,and COPD patients with depression disorder is a common comorbidities in COPD patients with COPD.(2)Gender,self-care ability,fear of disease,and fear of hospitalization costs are risk factors for COPD patients with depressive disorder.(3)Since COPD patients are prone to be complicated with negative emotions of depression disorder repeatedly,nurses should pay attention to the psychological state of patients and give appropriate psychological intervention on the basis of routine nursing,and formulate relevant nursing measures to effectively alleviate the clinical phenomenon of COPD patients with depression disorder.(4)Nursing staff should encourage patients to actively cope,reduce psychological burden and negative emotions,improve patients’ cognition of the disease,and help patients better fight against the disease.
