Objective: To summarize the clinical symptoms, laryngoscopy characteristics, and laboratory examination results of 22 patients with larynx tuberculosis, and to obtain the clinical symptoms, laryngoscopy characteristics, and laboratory examinations of larynx tuberculosis.Provide theoretical basis. Methods: From January 2015 to December 2018,22 patients with laryngeal tuberculosis treated in our hospital were selected as the research objects. The clinical characteristics, diagnostic methods and diagnosis of patients were summarized through a retrospective analysis of clinical data of patients.effect.Result:1.Clinical features: The clinical symptoms of patients with laryngeal tuberculosis are mainly hoarseness, accounting for 81.82%,followed by cough(72.73%),throat pain(68.18%),dysphagia(31.82%)、dyspnea(13.64%),and general symptoms(13.64%).Syndrome patients are mainly characterized by general weakness, fever, night sweats,or weight loss.2.Laryngoscopy: Through investigation of the lesions of patients,it was found that the lesions affected by larynx tuberculosis patients were mainly vocal cords.A total of 12 patients had lesions with vocal cords, accounting for 54.54%,followed by Cartilage(40.91%),ventricular zone(36.36%),and epiglottis(31.82%).Analysis of the results of laryngoscope diagnosis of 22 patients The results showed that the laryngoscopic lesion classification of patients with laryngeal tuberculosis was mainly inflammatory infiltrative type.A total of 10 patients had inflammatory infiltrating lesion type, accounting for 45.45%,followed by ulcerative necrosis(27.27%),granulation,the proliferative type(18.18%)and scar stenosis type(9.09%).The classification criteria refer to related research by other domestic scholars and clinical mirrors of patients.3.Performance under chest CT: Through chest CT examination of 22 patients, imaging results showed that of 22 patients with laryngeal tuberculosis,a total of 16 cases showed pulmonary tuberculosis, the diagnostic coincidence rate was 72.73%,indicating that chest CT can effectively achieve early screening of laryngeal tuberculosis, but It is easy to be misdiagnosed as tuberculosis and needs to be combined with other test results for diagnosis.4.Laboratory test results: 13 of the 22 patients with larynx tuberculosiswere positive for sputum smears, and the diagnostic coincidence rate was 59.09%.The diagnostic coincidence rate of sputum smear tests was significantly lower than the pathological diagnosis(P <0.05);22 A total of 16 patients with larynx tuberculosis had positive PPD results, and the diagnostic coincidence rate was 72.73%.The diagnostic coincidence rate of the PPD test was significantly lower than the pathological diagnosis(P <0.05).There were 14 of 22 larynx tuberculosis patients in total. The patient’s TP test result was positive, and the diagnostic coincidence rate was63.64%.The diagnostic coincidence rate of the TP test was significantly lower than the pathological diagnosis(P <0.05);15 of the 22 patients with laryngeal tuberculosis had positive sputum culture results. The coincidence rate was 68.18%.The diagnostic coincidence rate of sputum smear examination was significantly lower than that of pathological diagnosis(P <0.05).The diagnostic effect of different diagnostic methods was significantly different(P<0.05).Among them, the diagnostic coincidence rate of combined diagnosis It was significantly higher than other laboratory diagnostic methods of sputum smear,PPD,TP and sputum culture(P<0.05),and the diagnostic coincidence rate of combined diagnosis was not significantly different from pathological diagnosis(P> 0.05).After 12 months of treatment, all patients’ clinical symptoms improved or disappeared, and the effect was better. Conclusion: The clinical manifestations of patients with laryngeal tuberculosis mainly include hoarseness, cough, throat pain, dysphagia and dyspnea. The patient’s lesion classification is mainly inflammatory infiltration. The cultured combined examination can play a better diagnostic value in patients with laryngeal tuberculosis and provide an effective basis for the early screening and diagnosis of laryngeal tuberculosis. However,in view of the peculiarities of laryngeal tuberculosis, pathological diagnosis is still used in the clinic Diagnostic gold standard for TB patients. |
