Objective To analyze the prevalence and risk factors of depressive symptoms in hospitalized and community hypertensive patients.Methods From June 2019 to January 2020,154 patients with essential hypertension who were hospitalized in the Department of Cardiology,The Second Affiliated Hospital of Nanjing Medical University,and 114 patients with hypertension who have joined the chronic disease managementin in Mufushan Community Health Service Center were selected.The general conditions of hospitalized and community hypertensive patients were collected with designed questionnaire,and the blood biochemical indexes of hospitalized patients were collected at the same time.Patients’ depressive symptoms were assessed with patient Health questionnaire(PHQ-9).According to the score of 8 points on the scale,the patients were divided into depression and non-depression group.The data were analyzed by SPSS20.0,and the risk factors related to depression were analyzed by Logistic regression.Results Among the 154 hospitalized patients with hypertension,there were 86 males and 68 females.The average age was(65.70 ±12.79)years,of which 35 cases were assessed as depressed,accounting for 22.7% of the total number of cases.Among the 114 patients with hypertension(53 males and 61 females)in the community who participated in the management of chronic disease,the average age was(69.77 ±7.61)years,of which 22 cases were assessed as depressed,accounting for 19.3% of the total number of cases.There were significant differences in sex,age,marriage,course of hypertension,living condition and cerebrovascular disease between inpatients with hypertension with depressive symptoms and patients without depressive symptoms(P<0.05).Compared with the two groups of hypertension in the community,there were significant differences in sex,age,matrimony,education,family history of hypertension and age of onset(P<0.05).Among the biochemical indexes of hospitalized patients with hypertension,there were significant differences in folic acid,Hcy,Cr,ALB,HDL-C,FT3 and T3 levels(P < 0.05).Logistic regression analysis showed that the risk factors of depression in hospitalized hypertension patients were living alone(OR=25.124;95%CI:1.198~1527.041),elevated level of serum homocysteine(OR=1.234;95%CI:1.085~1.403),and risk factors of depressive symptoms in community hypertension patients were low education level(OR= 22.573;95% CI:2.821 ~180.620).Conclusion Living alone and the elevated level of serum homocysteine are the risk factors of hospitalized hypertensive patients with depressive symptoms,while education is the risk factor of community hypertensive patients with depressive symptoms.Clinicians,especially general practitioners,should pay attention to whether there are depressive symptoms in the treatment and management of patients with hypertension,discover them in time and take measures.
