Clinical Research Of Chinese Herbs Qing-xiao Lotion On Treatment Of Postoperative Complications Of Ⅳ Degree Annular Mixed Hemorrhoids Of Damp-heat Pouring Downward Syndrome Based On The Theory Of "Preventive Treatment Of Disease"

Objective: Under the guidance of "Preventive Treatment of Disease" theory,to observe the preventive and therapeutic effect of Chinese herbs Qingxiao lotion on postoperative complications of Ⅳ degree annular mixed hemorrhoids of damp-heat pouring downward syndrome,such as anal pain,anal edema,Pain and bleeding after postoperative defecation,wound healing rate and time etc.Methods: A total of 84 patients who met the criteria of diagnosis and inclusion were selected and randomly divided into the intervention group and the control group,42 cases in each,according to the random number table method.On the basis of preoperative and postoperative routine treatment,the intervention group was given fumigating and sitting baths with Chinese herbs Qingxiao lotion on the 3 days before surgery and the 2nd day to the 7th day after surgery,twice a day,once in the morning and evening,20min/ time,for a total of 9 days。The control group was treated with potassium permanganate solution(1:5000)2000ml,and was used for fumigation and washing treatment in the same method,twice a day,once in the morning and once in the evening.Observed,recorded and analyzed the each sample the anal pain and anal oedema circumstance on the preoperative 1 day,1st,3rd,5th and 7th days after operation,a maximum pain intensity(24 h Max)score within 24 hours of surgery,the point-in-time of the first postoperative pain(pain degree ≥4 points),pain and bleeding after postoperative defecate,the wound healing circumstance,by comparing before and after the intervention group itself and the comparison between control group and intervention group to evaluate the efficacy of the intervention.Results: Comparison of patients’ general conditions: there was no statistical significance in gender,age and course of disease(P<0.05),and there was no significant difference in baseline data,which was comparable.The research results are summarized as follows:(1)Comparison of anal pain: There was statistically significant difference between the two groups in the comparison of anal pain on the preoperative 1 day,1st,3rd and 5th days after surgery(P<0.05).But on the 7th days after surgery,there was no significant difference in anal pain(P>0.05).There was statistically significant difference in anal pain between the intervention group on the preoperative and 1st day after surgery(P<0.05).There were statistically significant differences in the score of maximum pain intensity within 24 hours and the point-in-time of the first postoperative pain(pain degree ≥4 points)between the two groups(P<0.05).(2)Comparison of edema of the anal margin: There was statistically significant difference between the two groups in the comparison of edema of the anal margin on the preoperative 1day,1st,3rd days after surgery(P<0.05).But on the 5th and 7th days after surgery,there was no significant difference in edema of the anal margin(P>0.05).(3)Comparison of postoperative defecation: There were statistically significant differences in the first,second and third anal pain after postoperative defecation between the two groups(P<0.05).But in the 4th anal pain after postoperative defecation,there was no significant difference(P>0.05).There were statistically significant differences in the first,second,third and fourth anal bleeding after postoperative defecation between the two groups(P<0.05).(4)Comparison of wound healing: There was statistically significant difference in wound healing time between the two groups(P<0.05).On the 14 th day after surgery,there was statistically significant difference in wound healing rate of the two groups(P<0.05).(5)Overall efficacy comparison: There was statistically significant difference in the overall efficacy comparison between the two groups after 14 days of treatment(P<0.05).Conclusion:On The Theory of “Preventive Treatment of Disease”,in the perioperative period,treating patients with Ⅳ degree circumferential mixed hemorrhoid of damp-heat pouring downward syndrome by giving fumigating and sitting baths with Chinese herbs Qingxiao lotion can achieve clear away heat and expel dampness,relieve swelling and pain,cool blood and hemostasis,quickly alleviate the symptoms of anal pain and edema of the anal margin,improve postoperative defecation,especially true in the early and middle stages,and can shorten the wound healing period.This method has the advantages of safety,low price,simplicity and less pain.This method has achieved good clinical effects,such as in the prevention and treatment of hemorrhoid postoperative complications and promoting the postoperative recovery of patients.It is worth popularizing and applying in a clinic.
