Objective:To investigate the effect of recombinant human prourokinase(rh-pro UK)on the prognosis of patients with ST segment elevation myocardial infarction(STEMI)complicated with high thrombotic load during emergency percutaneous coronary intervention(PCI).Methods:The clinical data of patients with high thrombus load who underwent emergency PCI due to STEMI in Lu’an people’s Hospital from January 2018 to January 2020 were analyzed retrospectively.According to the different drugs injected during the operation,43 patients were injected with tirofiban as the control group,and 45 patients with rh-pro UK were injected as the observation group.The TIMI myocardial perfusion grade(TMPG),TIMI flow grade,ST segment decline percentage(STR),left ventricular function,bleeding and major adverse cardiovascular events(MACE)during hospitalization were analyzed in the two groups.Results:1.Comparison of clinical baseline data:There was no statistically difference in age,sex,drinking and smoking between the two groups,and there was no significant difference in diabetes,hypertension,hyperlipidemia,criminal vessels and the number of vascular lesions between the two groups(P > 0.05).2.Comparison of myocardial tissue perfusion level after PCI:There was no statistically difference in TIMI blood flow grade between the two groups(P=0.123).There was significant difference in the proportion of TMPG grade3 and STR complete decline between the two groups(83.72% VS 97.78%,4.65% VS26.67%,P < 0.05).3.Comparison of left ventricular function index levels after PCI:The LVEDD and LVEF of the observation group after 1 month were better than those of the control group,and the difference was statistically significant(P=0.033,P=0.013,).4.Comparison of bleeding and MACE during hospitalization:There was no statistically difference in the incidence of MACE such as bleeding,angina pectoris and heart failure between the two groups(P>0.05).Conclusion:In patients with STEMI,intracoronary injection of rh-pro UK during emergency PCI may reduce the high thrombus load and facilitate the recovery of cardiac function,and there is no significant increase in bleeding and the occurrence of MACE.
