| Objective:To observe and evaluate the therapeutic effect difference between "Shugan Tiaoshen" acupoint catgut embedding method and acupuncture method in the treatment of generalized anxiety disorder(GAD)(liver and qi stagnation type),so as to preliminarily form the technical specification for acupoint catgut embedding treatment of GAD,and provide the basis for its clinical application and popularization.Methords:Sixty-six patients with GAD(liver and qi stagnation)admitted to the Affiliated Hospital of Gansu University of Traditional Chinese Medicine from October 2019 to December 2020 who met the inclusion criteria were divided into acupuncture group and embed cord emplacement group according to the nonrandomized contemporaneous control method according to the clinical reality,with 33 cases in each group.Both groups selected acupoints based on the theory of "Shugan Tiaoshen":Baihui,Shenmen(double),Neiguan(double),Taichong(double),Xinshu(double)and Ganshu(double).In the acupuncture group,the needles were kept for 30min,3 times a week,24 times in total.The embedding group was treated with "line body folding and rotary embedding method",once/2 weeks,4 times in total.The changes of TCM syndrome scores,Hamilton Anxiety Scale(HAMA),Stai-State Anxiety Scale(STAI)and Self-Rating Anxiety Scale(SAS)scores before and after treatment were compared between the same group and two groups,and the overall efficacy of the two groups was evaluated and compared according to the HAMA reduction rate.Results:(1)Overall efficacy comparisonIn the acupuncture group,10 cases were significantly effective,14 cases were effective,7 cases were ineffective,and the effective rate was 77.42%.In the embedding group,12 cases were significantly effective,15 cases were effective,5 cases were ineffective,and the effective rate was 84.37%.Both treatments had similar efficacy(P>0.05).(2)TCM syndrome score evaluationCompared with before treatment,the TCM syndrome scores of both groups were decreased after treatment(P<0.05),indicating that both treatments could improve the TCM syndrome manifestations.The improvement of TCM syndromes was similar between the two groups after treatment(P>0.05).(3)Comparison of HAMA total scores and sub-items between the two groupsCompared with before treatment,HAMA total scores in both groups were significantly decreased(P<0.05),indicating that both treatments could improve the symptoms of GAD.After treatment,there was no significant difference in total scores between the two groups(P>0.05),indicating that the two therapies had the same effect on improving patients’ anxiety.Further comparison of HAMA’s 14 sub-items showed that the scores of 13 sub-items in two groups were lower than those before treatment(P<0.05),but there was no statistical difference in the item of "fear"(P>0.05),indicating that both treatments could improve most of the anxiety and physical anxiety symptoms of patients.After treatment,the embedding group was better than the acupuncture group in improving "anxiety" and "insomnia"(P<0.05).(4)STAI integral comparisonSTAI is divided into SAI(state anxiety)and TAI(trait anxiety).Compared with before treatment,the SAI and TAI scores of both groups were significantly decreased(P<0.05),indicating that both treatments could improve the state anxiety and trait anxiety of patients.After treatment,the two treatments had the same effect on the improvement of state and trait anxiety(P>0.05).(5)Comparison of SAS pointsThe total score of SAS in both groups was significantly lower than that before treatment(P<0.05).After treatment,there was no statistical difference between the two groups(P>0.05),indicating that both treatments could equivalent improve the anxiety symptoms of patients.(6)Compared with the acupuncture group,the treatment frequency and time of the embedding group were less,and the medical expenses were lower,with statistical differences(P<0.05).(7)Follow-up one month after the end of treatment showed that 6 patients in the acupuncture group relapsed,with a recurrence rate of 25%.The recurrence rate in the embedding group was 3.7%,and the recurrence rate in the embedding group was significantly lower than that in the acupuncture group(P<0.05).Conclusion:(1)"Shugan Tiaoshen" acupuncture method and thread emburying method have good efficacy in the treatment of generalized anxiety disorder(liver and qi stagnation type),can significantly improve the TCM syndrome performance of patients,significantly reduce the anxiety and physical anxiety of patients,reduce the level of state anxiety and trait anxiety of patients."Shugan Tiaoshen" embedding method was better than acupuncture method in improving the sub-indexes of HAMA scale "anxiety" and "insomnia",and the recurrence rate was lower.(2)The treatment frequency,time and cost of acupoint catgut embedding are lower than that of acupuncture treatment,and it has the advantages of effective,excellent and low price. |
