The Study On The Correlation Between Peripheral Blood C3aR MRNA Expression And Serum C3a,C3aR,IL-1β Levels And The Progression Of Chronic Obstructive Pulmonary Disease

Abstract/Summary:
Objective:To investigate the peripheral blood C3aR mRNA expression and serum C3 a,C3aR,interleukin-1β(IL-1β)levels in patients with chronic obstructive pulmonary disease(COPD),and analyze its relationship with the airway inflammation and severity of COPD,providing new avenues for studying the pathogenesis of COPD and potential therapeutic targets for treatment.Methods:60 patients with acute exacerbation of COPD(AECOPD)were recruited from the Department of Respiratory Medicine,the Third Affiliated Hospital of Zunyi Medical University from July 2020 to January 2021.Based on the number of acute exacerbations during last year,AECOPD patients were divided into high-risk acute exacerbation group(≥2 moderate or ≥ 1 severe exacerbations requiring hospitalization in the previous 12 months)and low-risk acute exacerbation group(acute exacerbation occurred once or less in the previous 12 months and was not hospitalized);AECOPD patients were divided into respiratory failure group and non-respiratory failure group according to the blood gas analysis at the time of admission;Thirty hospitalized patients with stable chronic obstructive pulmonary disease(SCOPD)were involved in the stable group;30 healthy people from the medical examination center were included in the health group.The demographics information,venous blood samples,laboratory examination and lung function data of all participants were collected.Besides,the expression of C3aR mRNA in the peripheral blood was detected utilizing RT-PCR,and serum C3 a,C3aR,IL-1β levels were detected with ELISA.Moreover,correlations between lung function index FEV1%pred and peripheral blood C3aR mRNA expression,serum C3 a level,serum IL-1βlevel in the AECOPD group were calculated.Results:(1)There were no significant differences in age and gender of the three groups(P >0.05).Significantly decreased lung ventilation function indexes including FEV1,FEV1/FVC,FEV1%pred were found in the COPD group compared to healthy group(P <0.05);Significantly increased white blood cell count(WBC),neutrophil/lymphocyte ratio(NLR)and C-reactive protein(CRP)were detected in the COPD group compared to healthy group(P < 0.05).(2)Significantly increased peripheral blood C3aR mRNA expression and serum C3 a,C3aR,IL-1β levels were found in the AECOPD group compared to those in the SCOPD and healthy group(P < 0.05).And these measures in the SCOPD group were significantly higher than those in healthy group(P < 0.05).(3)Significantly increased peripheral blood C3aR mRNA expression and serum C3 a,C3aR and IL-1β levels were found in the high-risk group of AECOPD patients compared to the low-risk group(P < 0.05).(4)Significantly increased peripheral blood C3aR mRNA expression and serum C3aR,IL-1β levels were detected in the respiratory failure group of AECOPD patients compared to the non-combined respiratory failure group(P < 0.05).There was no significant difference in serum C3 a levels between the two groups(P > 0.05).(5)Correlation analysis showed peripheral blood C3aR mRNA expression and serum C3 a levels in the AECOPD group were positively correlated with serum IL-1β levels,and negatively correlated with FEV1pred%.Conclusion:(1)Significantly increased peripheral blood C3aR mRNA expression and serum C3 a,C3aR levels were found in COPD patients compared to healthy group,and the three indexes were positively correlated with serum IL-1β levels and negatively correlated with FEV1pred%.Therefore,the C3a-C3aR axis may be involved in airway inflammation in COPD patients and serve as a potential therapeutic target in the management of COPD airway inflammation.(2)Peripheral blood C3aR mRNA expression and serum C3 a level were positively correlated with serum IL-1β level in AECOPD patients,which suggests that the C3a-C3aR axis and IL-1β may mediate airway inflammation in AECOPD patients through the same inflammatory signal pathway.
