Comparison Of Different Endometrial Preparation Protocols For Frozen-thawed Embryo Transfer Cycle In Patients With Repeated Implantation Failure

Objective: The study compared the clinical pregnancy rate,multiple pregnancy rate,embryo implantation rate,abortion rate and live birth rate of patients,who haved been diagnosed with repeated implantation failure(RIF),in the frozen-thawed embryo transfer(FET)cycle with five different endometrial preparation protocols,so as guide to provide an individualized endometrial preparation protocol for the patients with RIF in clinical.Methods: This study retrospectively analyzed the clinical data of 110 patients with recurrent implantation failure(RIF)who received frozen thawed embryo transfer(FET)in the Reproductive Medicine Center of the Affiliated Hospital of Gui Lin Medical College from June 2014 to January 2020.Involving 135 transplant cycles,including 10 cycles of natural cycle group(NC group),24 cycles of ovulation induction group(OI group),11 cycles of hormone replacement therapy group(HRT group),30 cycles of half-does GnRH-a group(1.875 mg GnRH-a group)and60 cycles of full-does GnRH-a group(3.75 mg GnRH-a group).Groups were compared including iclinical pregnancy rate,multiple pregnancy rate,embryo implantation rate,abortion rate and live birth rate.Results: 1.There were no significant differences among five groups in age,body mass index,infertility type and basic sexual hormone levels(P>0.05).2.There were no significant differences among five groups in the number of blastocysts and cleavage stage embryos,excellent embryo rate and the endometrial thickness(P>0.05).3.The clinical pregnancy rate,embryo implantation rate and live birth rate of full-does GnRH-a group were slightly higher than those of other groups,but there was no statistical difference(P>0.05);the multiple pregnancy rate and abortion rate of the five groups had no statistical difference(P>0.05).Conclusions: 1.In FET,patients with RIF can obtain similar pregnancy outcomes by using natural cycle,ovulation induction cycle,hormone replacement cycle or down-regulated HRT cycle to prepare endometrium.2.In FET,natural cycle,ovulation induction cycle,hormone replacement cycle and down-regulated HRT cycle all can obtain favorable clinical outcomes for patients with RIF.In clinical practice,we should choose different endometrial preparation protocols according to different conditions of patients,and comprehensively consider the safety,economy and effectiveness of patients in assisted reproductive therapy,so as to realize individualized embryo transfer.
