Objective: Aim to investigate the correlation of uterine secretion metabolomics with the endometrial receptivity,and look out new predicting indexes of clinical outcome of IVF-ET.Methods: The uterine secretions of 95 women underwent frozen-thawed embryo transfer(FET)at the Reproductive Medicine Center of the PLA Clinical College of Anhui Medical University from January 2019 to December 2020 were collected in this study,while 285 women who underwent FET and did not undergo uterine secretion aspiration were severed as control at a ratio of 1:3.Meanwhile,the uterine secretions of32 pregnant women and 28 non-pregnant patients after FET were collected on the day of endometrium transformation for non-targeted metabolomics detection through UPLC-MS technology.Then,a variety of statistical methods were used to analyze pregnancy-related differential metabolites and related metabolic pathways.At the same time,the ROC curve was used to evaluate the diagnostic ability of differential metabolites in pregnancy.Results: 1.Aspiration of uterine cavity secretion on the day of endometrium transformation has no obvious adverse effect on the outcomes of FET.2.Metabolomics analysis of the uterine showed that there were a total of 65 differential metabolites between pregnancy women and non-pregnancy women.3.A total of 40 differential metabolites with diagnostic value were screened out through ROC curve analysis.4.There were significant differences in six metabolic pathways between pregnancy women and non-pregnancy women through metabolic pathways analysis.Conclusions: 1.Aspiration of uterine cavity secretion on the day of endometrium transformation to evaluate the endometrial receptivity is safe and non-invasive.2.A total of 40 differential metabolites with diagnostic value between pregnancy women and non-pregnancy women were screened out in this study.There might be abnormalities in amino acid metabolism,nucleotide metabolism,and glycerophospholipid metabolism of uterine secretions in non-pregnant women,which may be associated with pregnancy failure.Metabolomics analysis based on UPLC-MS could rich the information about the uterine secretions metabolomics,and be helpful for analyzing the causes of embryo implantation failure and evaluating endometrial receptivity and predicting the pregnancy outcome of IVF.
