The development of medical technology has brought new opportunities as well as new challenges to the diagnosis and treatment of lung cancer,especially the challenge of the rapid increase in medical expenses.The treatment cycle of lung cancer is long,which consumes a lot of health resources,leads to high medical expenses.Patients’ disease burden is heavy,and medical insurance fund runs under great pressure.This study collected the hospitalization information of all patients with lung cancer discharged from a tertiary hospital in Guizhou Province from 2012 to 2018.SQL Server 7.0software was used to clean and logically test the data,and the cases were selected according to the inclusion and exclusion criteria.SPSS 25.0 software was used for statistical analysis of patients’ basic characteristics and internal composition of hospitalization expenses,and multiple linear regression was used to fit path analysis model to analyze the influencing factors of hospitalization expenses.The data of hospitalization expenses with lung cancer in Guizhou province was analyzed to explore the effect of various influencing factors of hospitalization expenses and their ways of action,and to analyze their sources,which aims to provide theoretical reference for relevant departments to formulate medical expenses controlling measures,to reduce the burden of disease with lung patients,to curb the occurrence of "poverty caused by illness,return to poverty due to illness" phenomenon and to consolidate the achievements of poverty alleviation.The 6766 cases included in the study were mainly male patients and middle-aged and elderly patients,with an average age of(60.47±11.72)years.The completion rate of clinical pathway for lung cancer was only 23.90%,which was relatively low.The average length of hospital stay for lung cancer patients was 14.53 days(with a median of 11.00 days).The average drug cost was 10231.67 yuan(with a median of 4340.87 yuan).The average hospitalization expense was 29183.31 yuan(with a median of was 13456.97 yuan).With the change of the discharged year,the hospitalization expenses of lung cancer patients increase continuously,and the medical burden of medical insurance institutions,patients themselves and their families increased consecutively.Among the internal components of hospitalization expenses,drug costs accounted for the highest proportion,accounting for 35.68%,followed by diagnostic expenses.The internal components of hospitalization expenses needed to be further rationally optimized.The results of path analysis showed that there were many factors influencing the hospitalization expenses of patients with lung cancer,and the ways of action were complex.Among them,drug cost and length of stay were the key influencing factors,while medical payment method,surgery,discharge year and completion of clinical pathway were the main influencing factors.The keys to control the increase of medical expenses are to shorten the length of stay in hospital and reduce the cost of drugs.Medical institutions,medical insurance institutions and patients should cooperate with each other to promote the reform of payment methods.It is necessary to strengthen oversight of the medical insurance payment system,implement,refine and improve clinical pathways.
