Clinical Study Of “End-to-end Acupuncture” In The Treatment Of Motor Aphasia After Stroke

Objective:Observe the clinical efficacy of "end-to-end acupuncture" in the treatment of motor aphasia after stroke.And explore its role in the recovery of language functions,such as conversation,retelling,comprehension,naming and reading.Compare it with conventional acupuncture to judge whether the "end-to-end acupuncture" is worth popularizing.Methods:Picked the patients according to the standard,who got the aphasia after stroke and checked in the acupuncture and moxibustion department of the First Teaching Hospital of Tianjin University of TCM.The dates of admission were from January 2020 to March 2021.There were 72 patients altogether.Put them into two groups randomly.One was named the observation group and the other was the control group.Both of the two groups were cured with basic countermeasures,such as controlling blood pressure and blood glucose,anti-platelet aggregation,reducing blood lipid,improving brain metabolism,improving cerebral circulation and so on.Referring to the words about aphasia after stroke in "Acupuncture Therapy"(written by Yang Changsen),the control group was treated with basic acupuncture,while to the observation group was added the "end-toend acupuncture"(projection area of the affected language functional area,Yifeng,Shanglianquan,Jinjin,Yuye)and routine acupuncture treatment.All of the patients were cured for 30 minutes per day,6 times a week,for 2 weeks with a total of 12 times.The level of the damage in language’s function was examined by Aphasia Battery of Chinese(ABC),Traditional Chinese Medicine Symptom Score and the Boston Diagnostic Aphasia Examination(BDAE).Concerned the score of ABC as the main observation and curative effect index.The relevant data of scoring / grading of each group were collected to be analyzed by SPSS22.0 statistical software statistically.Results:1.Comparison of baseline data:Through statistical analysis,it is hard to distinguish the observation group from the control group in baseline aspects,including age,sex,type of disease,course of disease,ABC,TCM symptom Score and BDAE(P > 0.05).2.Comparison of clinical efficacy:After treatments,the proportion of observation group with 33 cases showed that significant validness: validness: improvement: no-validness was 13: 14: 5: 1(cases),meaning 96.97% of them performing well.In the other group with 34 cases,the proportion showed that significant validness: validness: improvement: no-validness was 1: 14: 17: 2(cases),meaning 94.12% of them performing well.It indicated that the observation group was better by the statistically significant difference(P < 0.05).3.Comparison of score improvement:After treatment,through the scores of the simple version of ABC,though both groups got benefits,the observation group got more.It was suggested that both "end to end acupuncture" and conventional acupuncture can improve the language function of patients,but the curative effect of "end to end acupuncture" was better.The aspects of improvements include conversation,retelling,comprehension,naming and reading.And the conversation,retelling,understanding and naming in observation group were better,while the improvement in reading was similar(P > 0.05).It showed that the "end-to-end acupuncture method" was superior to the conventional acupuncture method post-stroke motor aphasia.The Chinese symptoms were also relieved after treatment,and the validness of observation group was better(P<0.05).It mean that the "end-to-end acupuncture" is superior to the conventional acupuncture method in improving the symptoms of aphasia after stroke,such as difficult language,wrong language,mixed language,strong tongue and so on.The result estimated by the BDAE after treatment got the same conclusions,but the two acupuncture methods were similar in the result of BDAE(P>0.05),indicated that the improvement in degree of aphasia was same.Conclusion:1.Both conventional acupuncture and end-to-end acupuncture can promote the recovery of language function.2.The end-to-end acupuncture method has a more obvious effect on the recovery of patients’ language function,especially in spontaneous conversation,retelling,naming and understanding,which can further improve the quality of life of patients.3.Acupuncture therapy is simple for clinic to be popularized.
