Neutrophil Dysfunction In The Pathogenesis Of Acute Kawasaki Disease With Coronary Artery Lesions

Abstract/Summary:
Objective:To investigate the serum myeloperoxidase(MPO)and neutrophil elastase(NE)levels and their relationships with laboratory parameters and Z score of coronary artery in acute stage of children with Kawasaki disease(KD)and to explore the role of MPO,NE playing on the pathogenesis of KD.Methods:Fifty KD patients in acute stage of disease firstly diagnosed in Children’s Hospital of Chongqing Medical University from September 2020to March 2021 were included.According to coronary artery Z score,different numbers of coronary artery lesions,we divided these patients into several groups.Serum MPO,NE levels were measured by ELISA and were compared among the above groups.Then analyze the correlations between serum MPO,NE and laboratory parameter s,Z score of coronary artery in acute stage of KD patients.Results:1.Our study includs fifty acute stage KD patients.The patients without coronary artery lesions were non-coronary artery lesions group(Z<2,n=5),coronary artery dilation group(2≤Z<2.5,n=13),small co ronary artery aneurysms group(2.5≤Z<5,n=27),median coronary artery aneurysms group(5≤Z<10,n=5),36 children assigned as Health control(HCs group).The serum NE level between non-coronary group,coronary artery dilation group,small coronary artery aneurysms group,median coronary artery aneurysms group,HCs group were 28.38(15.35~44.93)ng/ml,32.88(19.26~46.43)ng/ml,41.68(30.94~51.72)ng/ml,65.22(47.80~77.05)ng/ml,29.64(21.33~57.70)ng/ml,respectively.The serum NE level of median coronary artery aneurysms group is higher than other groups,and non-coronary lesions group,coronary artery dilation group,HCs group had statistically significant difference when compared with median coronary artery aneurysms group(P<0.05).The serum MPO level among these groups had no statistically difference(P>0.05).2.Serum MPO/NE levels in children of five groups with different numbers of coronary artery involvement(none,one,two,three,four)had no statistically difference(P>0.05).When three arteries involved,the serum MPO were highest,and lowest when two were involved.Wh en four arteries involved,the serum NE levels were highest,and lowest when none was involved.3.Serum MPO level had positive linear correlation with ser um NE level.Serum NE level had positive linear correlation with Z LAD,Zmax,Both serum MPO,NE levels had negative linear correlation with hemoglobin(P<0.05).4.The area under the ROC curve of coronary artery aneurysms(CAAs)patients diagnosed by serum NE was 0.701(95%CI:0.553~0.850,P<0.05).The cut-off value was 29.46ng/ml,the sensitivity was 84.4%and specificity was 50%.Conclusions:The serum M PO/NE may be involved in the pathogenesis of KD and CALs,serum NE might have the potential to be a serological marker for CAAs.
