Tenascin-c And β-catenin Expression In Cholangiocarcinoma Tissues And Its Clinical Significance

Objective:The expression levels of Tenascin-c and β-catenin in cholangiocarcinoma tissues and adjacent tissues were detected.The correlation between the expression levels of both proteins and clinical pathological factors of patients,the correlation between the expression levels of Tenascin-c and β-catenin and the prognosis were analyzed by statistical methods,and the role of the two proteins in the occurrence and development of cholangiocarcinoma was obtained.Methods:Pathological wax blocks of 45 patients with cholangiocarcinoma who underwent radical resection in Department of Hepatobiliary Surgery from 2015 to2020 were collected.The experimental group was cholangiocarcinoma tissue,and the control group was adjacent tissue of the same patient(confirmed by pathologist under microscope without cancer cells).The expression levels of Tenascin-c and β-catenin in cholangiocarcinoma and adjacent tissues were detected by immunohistochemistry.The patients were grouped according to age,gender,tumor location,tumor size,differentiation,lymph node metastasis,TNM staging of cholangiocarcinoma guided by the third edition of the United States Joint Commission on Cancer(AJCC)guidelines 2019,lymph node metastasis and vascular invasion.Chi-square test was used to analyze whether there were differences in the expression levels of the two proteins in cancer tissues and adjacent tissues and the relationship between each case parameter and each protein expression level.Through the follow-up of patients,the relationship between the expression level of each protein and the prognosis was studied.Kaplan-Meier method was used to draw the survival time curve of patients.Single factor analysis was used to analyze the relationship between clinical case factors and prognosis.The variables with P < 0.05 were included in the COX regression model for multi-factor analysis,and the independent risk factors affecting the survival of cholangiocarcinoma were obtained.Finally,Spearman correlation coefficient was used to analyze whether the two protein expressions had a synergistic effect in the occurrence and development of cholangiocarcinoma.Results:1.Tenascin-c is mainly expressed in the cytoplasm of cancer cells,and a small part is expressed in the stroma;The expression level was different from the adjacent tissues,and the difference was statistically significant.Tenascin-c protein expression was correlated with TNM stage,lymph node metastasis and vascular invasion.It was not related to age,gender,tumor location,tumor size and CA-199 level.2.β-catenin is mainly expressed in cytoplasm and nucleus in cholangiocarcinoma tissues,while it is expressed in cell membrane in adjacent normal tissues.There were differences in the expression positions between the two,and the difference was statistically significant;β-catenin protein expression level and tumor differentiation degree;Lymph node metastasis,TNM stage and CA199 level were correlated,and the difference was statistically significant.It was not related to gender,age,tumor size and tumor growth location.3.Spearman rank correlation analysis showed that there was a positive correlation between the high expression of Tenascin-c and β-catenin in cholangiocarcinoma tissues,with a correlation coefficient(R = 0.304),suggesting that the two proteins may have a certain degree of synergy in the process of cholangiocarcinoma.4.Univariate analysis showed that the high expression of tumor size ≥ 2 cm,positive lymph node metastasis,TNM stage III-IV,and Tenascin-c and β-catenin in cholangiocarcinoma tissues were closely related to the poor prognosis of patients,statistical significance.Multivariate analysis showed that tumor ≥ 2 cm,high expression of Tenascin-c and β-catenin were independent risk factors affecting the survival of patients.Conclusion:The expression of TNC and β-catenin in cholangiocarcinoma tissues was higher than that in adjacent tissues,and the expression level was statistically significant.The high expression of TNC in cancer tissues was related to the positive lymph node metastasis,vascular invasion and high TNM stage,and the difference was statistically significant.The high expression of β-catenin in cancer tissues was related to low tumor differentiation,positive lymph node metastasis,high TNM stage,and CA199 ≥ 39 U / ml.High expression of both proteins is associated with poor prognosis and can be used as an independent risk factor for the prognosis of patients with cholangiocarcinoma.
