The Effect Of Health Belief Model On The Inhalation Adherence With Stable Chronic Obstructive Pulmonary Disease

Abstract/Summary:
Objective The purpose of this study is(1)to understand the adherence of inhalation in patients with Stable Chronic Obstructive Pulmonary Disease.(2)to analyze the influencing factors of the adherence of inhalation in patients with Stable Chronic Obstructive Pulmonary Disease.(3)to investigate the effect of nursing intervention based on Health Belief Model on the adherence of inhalation in patients with Stable Chronic Obstructive Pulmonary Disease.(4)to provide effective clinical nursing intervention guidance for improving the adherence of inhalation in patients with Stable Chronic Obstructive Pulmonary Disease.Methods In the first part of this study,the descriptive survey and influencing factors analysis of the adherence of inhalation in patients with Stable Chronic Obstructive Pulmonary Disease were conducted.To achieve them,the questionnaire survey was undertaken on 140 COPD patients who received treatment in the B and C areas of the respiratory department of a tertiary hospital in Inner Mongolia from November 2019 to July 2020.The research tools were Patient General Information Questionnaire,Morisky Medication Adherence Measure and Health Belief Model Scale.The first part of study can provide a basis to further nursing intervention. The second part of the study is the intervention effect of Health Belief Model on the adherence of inhalation in patients with Stable Chronic Obstructive Pulmonary Disease.In order to accomplish it,an experimental study was carried out.According to the inclusion and exclusion criteria,45 patients with poor inhalation adherence were selected from the patients with COPD who received treatment in the B and C areas of the respiratory department of a tertiary hospital in Inner Mongolia from August 2020 to October 2020 by convenience sampling method as the research objects.The B and C areas were randomly divided into the experimental group and the control group by the method of lottery.The experimental group was carried out the nursing intervention based on Health Belief Model,while the control group was carried out with responsibility nursing intervention.However,patients in both groups were followed up for 2 months.Baseline data and follow-up data,such as Patient General Information Questionnaire,Morisky Medication Adherence Measure and Health Belief Model Scale were collected by on-site methods.The collecting ways include fill in the questionnaire on the spot,making phone calls or chatting in We Chat.In these ways,the adherence of inhalation results of the two groups were got.Results In the first part of the study,the data of 140 subjects were collected from the descriptive survey and influencing factors analysis of the adherence of inhalation in patients with Stable Chronic Obstructive Pulmonary Disease.The scores for inhalation adherence with Stable Chronic Obstructive Pulmonary Disease ranged from 0.25 to 8,and the total score is 3.00(1.50,6.88).The scores of five dimensions of health beliefs,which include perceptions of vulnerability,perceptions of severity of disease,perceptions of benefits and barriers,self-efficacy and behavioral clues were 26.93±7.62,30.18±2.58,18.11±7.00,15.97±5.11,34.02±12.56,respectively.The influencing factors of inhalation adherence with Stable Chronic Obstructive Pulmonary Disease were self-efficacy,perceptions of benefits and barriers,perceptions of vulnerability,perceptions of severity and behavioral clues,respectively.Self-efficacy has the strongest effect on inhalation adherence,the other factors were perceptions of benefits and barriers,perceptions of vulnerability,perceptions of severity and behavioral clues.Besides,the standardized regression coefficients of all influencing factors were positive.In the second part of the experimental study,data were collected from 90 subjects,which include 45 in both the experimental group and the control group.The results showed that:(1)The adherence of inhalation in the two groups was affected by both the intervention measures and time,and the trend of adherence with inhalation of both the experimental group and the control group varied with time.(2)The adherence of inhalation in experimental group was significantly higher than that in control group at 1 month and 2 months after intervention(P<0.05).(3)The adherence of inhalation in the two groups at 1 month and 2 months after intervention was significantly higher than that before intervention(P<0.05).Conclusions (1)Patients with stable Chronic Obstructive Pulmonary Disease have poor adherence with inhalation.(2)The adherence with stable Chronic Obstructive Pulmonary Disease patients is affected by the level of health beliefs of patients,and medical staff should pay more attention to the health beliefs of COPD patients.(3)Nursing intervention based on the Health Belief Model has a good effect in improving the adherence of inhalation in COPD patients with Stable Chronic Obstructive Pulmonary Disease.
