| 【Objective】(1)To establish the safety management evaluation items for"Helpless"patients with acute alcoholism,and compile checklist for the Safety Management of Patients with"Helpless"in Acute Alcoholism.(2)To explore the clinical application of checklist for the Safety Management of"Helpless"Patients in Acute Alcoholism.【Methods】(1)By consulting the literature,sharing clinical experience,and setting up a project expert group,formulate the first draft of checklist for the Safety Management of"Helpless"Patients in Acute Alcoholism,and using Delphi Expert Consultation Method(Delphi Method)to screen the items and form the final draft of checklist for the Safety Management of"Helpless"Patients in Acute Alcoholism.(2)Using the similar experimental research method,select the"Helpless"Patients in acute alcoholism in the emergency department of a class 3 first level general hospital in a city from July 2018 to December 2018 and July 2019 to December 2019,which were taken as the research object,with a total of 357 cases.The control group(181 cases)used routine management method care from July 2018 to December 2018,and the experimental group(176 cases)used checklist for the Safety Management of"Helpless"Patients in Acute Alcoholism from July 2019 to December 2019.To compare and analyze the incidence of adverse events/the incidence of unharmful abnormal events of the two groups of patients during treatment in the emergency room and the patients’satisfaction with nurses.【Results】(1)Checklist for the Safety Management of"Helpless"Patients in Acute Alcoholism’s Delphi expert consultation coordination coefficient is 0.025(P<0.00),the coefficient of variation is 0.00~0.128,and the full score ratio is>40%.The degree of recognition and consistency are high,the opinions tend to be consistent,and the coordination is good.(2)The incidence of adverse events/The incidence of unharmful abnormal events in the experimental group was significantly lower than that in the control group.Among them,the incidence of financial disputes decreased from 35.4%to 8.5%before implementation,the difference was statistically significant(χ~2=38.7,P<0.001);The safety rate of patients decreased from 55.8%before implementation to 27.3%,the difference was statistically significant(χ~2=43.8,P<0.001);The incidence of medical safety decreased from 75.1%to 48.3%before implementation,the difference was statistically significant(χ~2=28.7,P<0.001);The incidence of medical disputes decreased from 7.7%to 1.1%before implementation,the difference was statistically significant(χ~2=11.5,P<0.001);The incidence of nursing disputes decreased from 4.42%before implementation to 0.57%,the difference was statistically significant(χ~2=11.3,P<0.05);The incidence of safety management decreased from 13.3%before implementation to 1.7%,the difference was statistically significant(χ~2=17.4,P<0.001).(3)The satisfaction of patients in the experimental group(95.5%)was higher than that in the control group(74%),the difference was statistically significant(P<0.001).【Conclusions】(1)Checklist for the Safety Management of"Helpless"Patients in Acute Alcoholism compiled by this research institute has a high degree of expert recognition and consistency,and good coordination.It can be used as a clinical tool for the safety management of"helpless"patients with acute alcoholism in emergency.(2)Checklist for the Safety Management of"Helpless"Patients in Acute Alcoholism reduces the incidence of adverse events/the incidence of unharmful abnormal events in patients,improves patients’satisfaction with nurses,and can be popularized and applied in clinical practice. |
