Objective: To investigate the effect of the time of the second stage of labor on the blood gas of the umbilical artery in neonates under the new labor analgesia,and to provide the basis and reference for better improving the pregnancy outcome.Methods:A total of 757 first-term pregnant women with full term,head position and single fetus who delivered in Gaocheng People’s Hospital from January 2018 to December 2018 were selected as the research subjects(excluding abnormal fetal position,elective cesarean section and menstrual women).After opening the uterine mouth 3CM wide,continuous fetal heart electronic monitoring and continuous epidural painless delivery anesthesia were given.According to the time of the second stage of labor,they were divided into two groups: group 1(n=584 cases): the second stage of labor≤3 hours,and group 2(n=173 cases): > ≤3 hours and ≤4 hours.Group 1 can be divided into group A(Class I graphics)and group B(Class II graphics)according to the three types of fetal heart electronic monitoring graphics.The two groups can be divided into C group(Class I graphics)and D group(Class II graphics)according to the three types of graphics of fetal heart electronic monitoring.The blood gas index(pH,lactic acid and alkali surplus),1 min Apgar score and other indexes of neonates in each group were compared.Neonatal NICU occupancy rate.Results:1.The pH value and 1 min Apgar score of neonatal umbilical artery blood gas in group 1 were significantly higher than those in group 2,and the residual levels of lactic acid and alkali were significantly lower than those in group 2,with statistical significance(P<0.05).2.The pH value and 1 min Apgar score of neonatal umbilical artery blood gas in group A were higher than those in group B,and the residual levels of lactic acid and alkali were significantly lower than those in group B,with statistical significance(P<0.05).3.The pH value and 1 min Apgar score of neonatal umbilical artery blood gas in group C were higher than those in group D,and the residual levels of lactic acid and alkali were significantly lower than those in group D,with statistical significance(P<0.05).4.The pH value and 1 min Apgar score of neonatal umbilical artery blood gas in group A were higher than those in group C,and the residual levels of lactic acid and alkali were significantly lower than those in group C,with statistical significance(P<0.05).5.The pH value and 1 min Apgar score of neonatal umbilical artery blood gas in group B were higher than those in group D,and the residual levels of lactic acid and alkali were significantly lower than those in group D,with statistical significance(P<0.05).6.APGAR1-minute score of neonates in group A was higher than that in group B(P<0.05);Apgar score in group C was higher than that in group D(P<0.05).Apgar scores of newborns in groups A and C and B and D were not statistically significant(P>0.05).7.The NICU occupancy rate of the A1 group was lower than that of the A2 group,and that of the B1 group was lower than that of the B2 group,with statistical significance(P<0.05).The NICU occupancy rate of the A1 group was lower than that of the B1 group,and the NICU occupancy rate of the A2 group was lower than that of the B2 group,with statistical significance(P<0.05).Conclusion:1.The prolongation of the second stage of labor can decrease the pH value of blood gas index and 1 min Apgar score of umbilical artery,and increase the residual levels of lactic acid and alkali.2.No matter whether the second stage of labor is prolonged or not,once the second pattern appears,the pH value of blood gas index and 1 min Apgar score of neonatal umbilical artery will decrease,and the residual levels of lactic acid and alkali will increase.
