ObjectiveBy measuring relevant indicators of thrombosis: thrombus precursor protein,P-selectin,D-dimer,tissue-type plasminogen activator,plasminogen activator inhibitor-1,von willebrand factor,nitric oxide,analysis of first-episode schizophrenia patients without medication groups before and after treatment,the course of five years or more group and healthy control group,the difference between clarify schizophrenia disease and processes and antipsychotic medication effects on thrombosis.MethodFirst-episode unmedicated patients with schizophrenia who were admitted to the outpatient department or ward of shenzhen kangning hospital from May 2019 to May 2020 were selected as the first-episode group,and the duration of schizophrenia ≥ 5 years in the hospital treatment group was selected during the same period.At the same time,the age and gender of the first-episode unmedicated patients group were matched,and healthy people without mental illness and no history of mental illness in their direct blood relatives were selected from the staff of kangning hospital of shenzhen as the control group.Fasting venous blood was collected from all the enrolled subjects in the early morning of the second day after enrollment.The blood routine,liver and kidney function indicators,four coagulation indicators,blood glucose and lipid indicators were determined by the laboratory.In addition,ELISA was used to detect thrombus precursor protein,P selectin,d-dimer,tissue plasminogen activator,tissue plasminogen activator inhibitor-1,von willebrand factor and nitric oxide.The first-episode unmedicated patients group was given antipsychotic treatment upon admission,gradually increasing to an effective dose.After 4 weeks of treatment,relevant indicators were also measured.SPSS 25.0 statistical software was used for statistical analysis of the measured experimental data,Mann-whitney U test was used to compare the first-episode groups and healthy groups,the results are expressed as median(quartile spacing).Wilcoxon test was used to compare the before and after treatment in the case group.and the results are expressed as median(quartile spacing).P≤0.05 was considered statistically significant in comparison between the two groups,and bilateral P≤0.05 was considered statistically significant in comparison between the three groups.The counting data were tested by chi-square,and the results were expressed by the percentage of the number of examples.Results1.The median plasma values of Tp P,s P,NO,PAI-1 and PAI-1/t-PA in the first-episode groups were(15.61,2.78,89.35,28.61 and 3.12)higher than the healthy control group(5.59,1.18,50.70,15.69 and 2.00),the differences were statistically significant(P<0.05).The median plasma t-PA and v WF in the first-episode groups were(9.59 and 383.18)lower than the healthy control group(10.20)and(403.33),the difference was not statistically significant(p>0.05).2.The median plasma values of Tp P,s P,NO,t-PA and v WF in the post-treatment group were(10.54,1.87,68.16,38.86)and(291.08),respectively,which were lower than those in the first-episode groups(15.61,2.78,89.35,63.90 and 383.18),respectively.the differences were statistically significant(P<0.05).The median plasmas of PAI-1 and D-dimer in the post-treatment group were(21.99)and(300.00)lower than that the first-episode groups(28.61)and(340.00),and the difference was not statistically significant(p>0.05).3.The median plasma values of Tp P,s P,D-dimer,NO and PAI-1 in the chronic case group were(13.13,2.53,270,83.16 and 25.78),which were lower than those in the first-episode groups of(15.61,2.78,340,89.35 and 28.61).The median plasma t-PA and v WF values in the chronic case group were(12.34)and(471.45)higher than those in the first-episode groups of(9.59)and(383.18),the difference was not statistically significant(p>0.05).The median plasma values of PAI-1/t-PA in the chronic case group was(2.32)lower than in the first-episode groups(3.12),the difference was statistically significant(p>0.05).4.The median values of Tp P,NO,and PAI-1 in the chronic case group were(13.13,83.16,and 25.78)higher than the post-treatment group(10.54,68.16,and 21.99),The median values of D-dimer in the chronic case group was(270)lower than the post-treatment group(300),the difference was not statistically significant(p>0.05).The median s P,t-PA,and v WF values in the chronic case group were(2.53,12.34,and 471.45)higher than the group after treatment(1.87,5.83,and 291.08),The median plasma values of PAI-1/t-PA in the chronic case group was(2.32)lower than those in the post-treatment group(3.47),the difference was statistically significant(P<0.05).Conclussion1.The plasma Tp P,s P,NO and PAI-1 levels were higher in the first episode unmedicated schizophrenia group than in the healthy group,suggesting that the disease itself would lead to changes in thrombotic indicators.2.After short-term treatment,plasma Tp P,s P,NO,t-PA and v WF decreased in patients with first-episode schizophrenia,suggesting that short-term antipsychotic treatment would affect thrombus function.3.The plasma levels of s P,t-PA and v WF in patients with schizophrenia under long-term hospitalization were higher than the post-treatment group,suggesting that both the course of the disease and drug therapy could affect thrombus levels. |
