Objective: The purpose of this study is to explore the therapeutic effect of surfactantreplacement therapy(SRT)on RDS of premature infants in the Qinghai-Tibet Plateau,and whether there is any difference in the therapeutic effect at different altitudes.Method: This study is a multi-center retrospective study.We collected and screened reasonably clinical data of premature infants RDS from 10 hospitals in the Qinghai-Tibet Plateau from 2015 to 2017.According to treatment of SRT and the altitudes after birth,we divided them into groups and analyzed their therapeutic effect using logistic regression.Furthermore,we investigated the improvement of blood gas and prognosis of two groups of RDS infants after SRT at different altitudes to compare therapeutic effect.Results: This study finally included 337 cases of premature infants with RDS.Whether high or ultra-high altitude,the mortality rate of children with RDS in the SRT group was significantly lower than that in the non-SRT group(both p<0.05,OR<1).The effect of SRT on premature infants with RDS in the high altitude(p = 0.0043)was better than that in the ultra-high(p = 0.0194).Similarly,the improvement of Pa O2/Fi O2 and p H of infants in the high altitude was significantly better than that in the ultra-high altitude(all p<0.01)after SRT.Conclusion: SRT is effective for RDS in premature infants in plateau area.However,in plateau area,as the altitude of the area where the infant is born increases,the efficacy of SRT will decrease.
