Objective:To do a retrospective study of the prognostic value of serum cystatin C level in heart failure with preserved ejection fraction(HFp EF).Methods:This study was a retrospective study.1767 HFp EF patients in our hospital were collected and divided into four groups according to the serum cystatin C level.The differences in gender,age,disease history,laboratory indexes and disease severity were compared among the four groups.The survival curve was drawn by Kaplan Meier method,and the survival rate was compared by using univariate and multi-factor COX.Results:There were significant differences in cardiac function classification(P < 0.001),in-hospital mortality(P = 0.001)and all-cause mortality(P < 0.001)among the four groups.The higher the level of cystatin C,the greater the cardiac function classification,the higher the mortality.Kaplan Meier survival curve showed that with the increase of serum cystatin level,the mortality of patients gradually increased.In univariate Cox survival analysis,Cystatin C(HR=1.134,95%CI 1.038-1.239,P=0.005),NT-pro BNP(HR=1.542,95%CI 1.217-1.953,P < 0.001),admission systolic blood pressure(HR=0.822,95 %CI 0.679-0.997,P=0.046)is a factor predicting all-cause death after discharge from hospital,while other clinical indicators are meaningless.In a multivariate analysis,after adjusting for other related factors,Cystatin C can still be used as an indicator to predict all-cause death after discharge(HR=1.163,95%CI 1.045-1.293,P=0.005).It proves that serum cystatin C is an independent risk factor that can predict the prognosis of patients with HFp EF in either single-factor or multi-factor COX regression analysis.Conclusions:High serum cystatin C levels may mean worse cardiac function and higher mortality.Serum cystatin C is a new method to predict the prognosis of patients with HFpEF.
