The Relative Risk And Chinese Medicine Therapy Of Diabetes Mellitus And Osteoporosis

ObjectiveTo analyze the relative risk(RR)of type 2 diabetes mellitus(T2DM)and osteoporosis(OP).Then to explore the rule of Chinese Medicine(CM)in the treatment of DM combined with OP based on data-mining and assess the clinical studies qualities of specific prescriptions,so as to provide references for clinical study,prevention and treatment of DM combined with OP.MethodsThe data of FNF patients were collected from the First Affiliated Hospital of Guangzhou University of Chinese Medicine from January 1,2015 to December12,2017.Then we assess the RR values of T2 DM combined with OP in FNF patients.We also evaluated the RR values of T2 DM patients combined with OP in different parts of body from CNKI,VIP,Wanfang,and Pubmed.At the same time,we data mining the prescription of OP combined with DM from the well-known CM professor Huang Feng of Guangdong province,CNKI,VIP,Wanfang,and Pubmed using Traditional Chinese Medicine inheritance support system(TCMISS).At last,quality evaluation the clinical studies of specific drug.Results1.In the patients of FNF,about 267 patients with OP(267/278),60 patients with T2DM(60/278),the proportion of patients with T2 DM and OP was 60/278(21.58%).Clustering results showed that 105 patients in the first group,about 59 patients have T2 DM,all of them have OP,with the highest risk factor score(an average of 2.61 points).In FNF patients,the RR value of T2 DM combined with OP is higher than non-T2DM(RR=1.05).A meta-analysis of previous clinical studies found that the prevalence of OP in patients with T2 DM was 1.57 times that of normal patients when assessed the BMP of hip and lumbar position[RR=1.57,95% CI(1.09,2.27)](P<0.05).2.A total of 80 prescriptions were included,six drugs were most frequently used,including huangqi[stragalus membranaceus(Fisch)Bunge],dangshen(Radix Salviae Miltiorrhizae),yinyanghuo(Epimediumdavidii Franch),shudihuang[Rehmannia glutinosa(Gaetn)Libosch ex Fisch et Mey],shanyao(Dioscorea opposita Thunb),shanzhuyu(Cornusofficinalis).34 herb-groups were frequently used.Huangqi,shanyao,dangshen,shudihuang,gusuibu(Davallia mariesii Moore ex Bak),yinyanghuo,shanzhuyu,buguzhi(Psoralea corylifolia Linn),niuxi(Achyranthes bidentata Blume),duzhong(Eucommia ulmoides),gouqi(Lycium barbarum L),shengdihuang(Rehmannia glutinosa Libosch)were in the core drugs correlation network,which belongs to the Zuogui pill and Zicui decoction.The channels of Chinese herbs were liver,kidney and spleen.The qi and taste is given priority to with warming and sweet.At the same time,8 new drug combinations were excavated.3.The reproducibility of clinical specific prescriptions is poor,and there is an uncertainty bias in selective reporting,blinding of participants and personnels,blinding of outcome assessments.ConclusionThe RR value of T2 DM combined with OP is high.The DM combined with OP treating from insufficiency of kidney essence deficiency,and yin deficiency with internal heat.Specific methods including enrich the kidney and replenish essence,strengthening sinews and bones,engender fluid and moisten the lung.Zuogui pill and Zicui decoction were advocating to treating OP combined with DM.At present,the quality of clinical study on Chinese medicine is poor and repeatability is bad,which are required high-quality trials to further confirm these results.
