Abstract/Summary:
Part oneObjective: To observe the effects of different weight management methods on reproductive endocrine,energy and lipid metabolism in obese PCOS patients.Methods: from September 2017 to December 2018,118 obese PCOS patients(BMI ≥25)were randomly divided into experimental group and control group(59 cases in each group).The control group was treated with diet and exercise,while the experimental group was treated with diet and metformin and Diane-35,Six months later,the related indexes of the two groups were compared.Results: the levels of hormone and metabolism in the experimental group and the control group before and after weight loss were effectively improved,and the comparison indexes(BMI,LH,TT,FINS,total cholesterol,triglyceride)in the group were statistically significant(P < 0.05).Conclusion: the two different weight management models can significantly reduce the weight of obese PCOS patients in a certain period of time,reduce the fasting insulin and testosterone levels,reduce the symptoms of insulin resistance and hyperandrogenemia,and effectively improve the endocrine,glucose and lipid metabolism disorders of obese PCOS patients.Part twoObjective: To explore the effect of different weight loss methods on pregnancy outcome of obese PCOS patients.Methods: patients who received IVF-ET treatment in the reproductive center of Qingdao women’s and children’s Hospital from September 2017 to May 2019 were selected and divided into obese PCOS unreduced group(group A: no weight loss behavior in any form within half a year),obese PCOS unreduced group(group B: no weight loss behavior in any form(diet,exercise,medicine,surgery,etc.)Obesity PCOS diet and exercise reduction group(group B: those who voluntarily received diet and exercise treatment for weight management and successfully lost weight),obesity PCOS drug reduction group(Group C: those who voluntarily received diet,metformin and Diane-35 combined with drug treatment for weight management and successfully lost weight),non PCOS non obesity group(Group D: those who needed assisted reproductive technology only because of tubal infertility).In strict accordance with in vitro fertilization embryo transfer process to complete pregnancy.Results: compared with the obesity group,the follicles,eggs and embryos of the two groups were better than those of the obesity group,and the clinical pregnancy rate of the two groups were better than those of the obesity group.Conclusion: the two weight loss modes can increase the sensitivity of patients to GN drugs,shorten the duration and dosage of GN.The diet and exercise weight loss methods can improve the rate of oocyte retrieval and excellent embryo rate,improve the pregnancy outcome of obese PCOS patients,and increase the embryo implantation rate and the pregnancy rate.The diet and exercise weight loss methods have better effect.
