| Objectives The objective of this study is to analyze the influence factors of Acute Stress Disorder(ASD)in patients with Liver cirrhosis complicated with Upper gastrointestinal bleeding,and to provide basis for clinical nursing.Methods The grouping of patients from October 2019 to August 2020 with Liver cirrhosis complicated with Upper gastrointestinal bleeding in Kailuan General Hospital were selected and evaluated according to the results of Stanford Acute Stress Response Questionnaire(SASRQ).197 patients with acute stress disorder of liver cirrhosis complicated with upper gastrointestinal bleeding were selected into the case group,while197 patients with non-acute stress disorder of liver cirrhosis complicated with upper gastrointestinal bleeding were selected into the control group.The hospital Anxiety and Depression Scale(HADS),Perceived Social Support Scale(PSSS),Connor-Davidson Resilience Scale(CD-RISC),Visual Analogue Scale for Pain(VAS),Self-care Ability Assessment Scale(Barthel Index)and self-made questionnaires on general situation,disease situation,treatment situation,role function of medical staff,hospital environment and so on were used to evaluate the influencing factors of the two groups of patients.Excel software was used to establish the database,spss23.0 was used for statistical analysis,Chi square test was used for single factor analysis,and Logistic Regression Analysis was used for multiple factors analysis.Results 1 Univariate analysis of Acute stress disorder in patients with Liver cirrhosis complicated with Upper gastrointestinal bleeding showed that the facters in the case group and the control group,which contain hematemesis fever,hematemesis volume before admission,hematochezia frequency after admission,palpitation,fatigue,edema of both lower limbs,sleep quality,difficulty in falling asleep,dread,anxiety,fear,tension,burden of care,health education for nurses,bed trouble,fasting water trouble,social support,resilience level,anxiety and depression,self-care level,has differences and the differences were statistically significant(P<0.05).2 According to the Multivariate Logistic Regression Analysis of Acute stress disorder in patients with Liver cirrhosis complicated with Upper gastrointestinal bleeding.The results showed that anxiety and depression(OR=30.188),fatigue(OR=13.699),bleeding in the stool after admission(OR=3.808)and care burden(OR=3.788)were the risk factors of acute stress disorder in patients with liver cirrhosis complicated with upper gastrointestinal bleeding(P<0.05).Psychological resilience(OR=0.108),social support(OR=0.219),sleep quality(OR=0.541)and health education by nurses(OR=0.793)are the protective factors of ASD in patients with liver cirrhosis complicated with gastrointestinal bleeding(P<0.05).Conclusions The risk factors of ASD in patients with liver cirrhosis complicated with upper gastrointestinal bleeding include the number of hematochezia after admission,fatigue symptoms,care burden,anxiety and depression level.The protective factors of ASD in patients with liver cirrhosis complicated with upper gastrointestinal bleeding are:sleep quality,health education by nurses,social support level and psychological resilience level.It can provide the basis for early intervention.Figure1;Table15;Reference 163... |
