| Objective: Objective to investigate the correlation between smoking and the level of endothelial cell-derived microparticles in patients with acute myocardial infarction,It provides scientific basis for revealing the mechanism of smoking mediated acute thrombosis in blood vessels.Methods:Patients with acute myocardial infarction(AMI group,n = 40)and stable coronary heart disease(CAD group,n = 40)who visited the Department of Cardiology of our hospital from January 2020 to December 2020 were included.The levels of EMPs(CD31 +)were detected by flow cytometry,and the clinical biochemical indexes were detected.Results:The levels of systolic blood pressure(121.25±17.81)、 ALT(20,42)、 CHOL(4.30±1.11)、 LDL(2.71±0.89)、 CK(70.75,849.75)、CKMB(6.825,122)、CTn T(0.19,2.54)、CTn I(0.35,20.95)、EMPs(50.46±19.11)、 smoking index(328.57 ± 144.54)in AMI group were significantly higher than those in CADgroup(P<0.05).Univariate logistic regression analysis showed that systolic blood pressure,CHOL,LDL,CK,CKMB,troponin I,endothelial microparticles and smoking were the risk factors of AMI,and the difference was statistically significant(P < 0.05).Further multivariate logistic regression analysis showed that after adjusting for age,EMPs,smoking and LDL were independently associated with AMI.ROC curve was drawn to analyze the diagnostic value of EMPs in predicting AMI.AUC of EMPs and AMI was 0.745,95% Cl: 0.632-0.875,P < 0.001.The level of EMPs in smoking group(54.50 ±11.40)was significantly higher than that in non-smoking group(36.90 ± 9.30),and the difference was statistically significant(P < 0.05).Pearson correlation analysis showed that smoking index was positively correlated with EMPs and syntax scores,and the differences were statistically significant(P < 0.05).Univariate logistic regression nalysis with EMPs as the dependent variable showed that low HDL,low EF and smoking were the risk factors for the increase of EMPs.Further multivariate logistic regression analysis showed that low HDL and smoking were independent risk factors for EMPs.Conclusion:Smoking can promote the level of EMPs;smoking and the increased level of endothelial cell-derived particles are independent risk factors for acute myocardial infarction,suggesting that smoking-induced endothelial cell particle release may be one of the reasons that smokers are more likely to develop acute myocardial infarction. |
