| ObjectiveObstructive sleep apnea syndrome(OSAS),also known as obstructive sleep apnea hypopnea syndrome(OSAHS),is a kind of sleep disorder of breathing. Its clinical manifestations are daytime sleepiness, nighttime snoring and apnea.Patients with OSAHS may suffer from nocturnal hypoxia and hypercapnia due to apnea.Recurrent episodes can lead to complications such as diabetes, hypertension, coronary heart disease and related cardiovascular and cerebrovascular diseases, and even sudden death at night.Inflammatory process and oxidative stress reaction caused by hypoxemia can lead to vascular endothelial dysfunction and increase the incidence of related diseases. This paper evaluates the factors affecting sleep quality of OSAS patients by comparing and studying the related indicators such as sleep latency,AHI,oxygen desaturation index, and so on.The related factors of blood pressure, glucose and lipid of the subjects were measured and discussed. The relationship between polysomnography and vascular endothelial function in patients with OSAS can be concluded by the influence of cardiovascular and cerebrovascular diseases.MethodsFrom January 2018 to December 2018,22 patients with obstructive sleep apnea hypopnea syndrome diagnosed by sleep electroencephalogram were selected as experim-ental group and 17 patients with normal sleep as control group. The basic data of age,BMI,blood pressure, neck circumference and abdominal circumference were obtained and measured.Blood glucose and lipid levels were measured. In addition, the basal diameter(D0)of brachial artery was measured by UNEXEF38 G vascular endothelial detector in Yunax,Japan.Brachial artery diameter(D1)after reactive congestion and brachial artery diameter(D2)after nitroglycerin administration.According to the formula FMD=[(D1-D0)/D0]*100%,NID=[(D2-D0)/D0] *100%,the ratio of blood flow-mediated and oral nitroglycerin-mediated vasodilation was calculated respectively. The main factors affecting the rate of vasodilation were compared with the control group, and the pathological changes of vascular endothelial function in patients with obstructive sleep apnea-hypopnea syndrome were evaluated.ResultsThere were 15 males and 7 females in 22 patients with obstructive sleep apneahypopnea syndrome. The sex ratio was 2.14:1.Among 17 healthy subjects,11 were males and 6 were females, with a sex ratio of 1.83:1.The average age of 22 patients with obstructive sleep apnea-hypopnea syndrome(OSAHS)was 50.32 S 4.63 years, and the average age of 17 healthy subjects was 49.96 S 3.83 years. The sleep quality level of patients with obstructive sleep apnea-hypopnea syndrome decreased significantly, the frequency of limb movement, the frequency of micro-awakening and the proportion of shallow sleep increased, the difference was statistically significant;AHI and body mass index, blood pressure, neck circumference, abdominal circumference and other factors were analyzed, the results showed that AHI and body mass index, abdominal circumference, neck circumference and triglyceride water.Equality has a close relationship. It is also associated with diastolic, systolic blood pressure. There was no significant difference in the basal diameter of the right brachial artery and the internal diameter of the brachial artery after reactive congestion between the experimental group and the control group(p > 0.05),but there were significant differences in the change rate of the internal diameter of the brachial artery after reactive congestion, the change rate of the internal diameter of the brachial artery after taking nitroglycerin and the change rate of the internal diameter after taking nitroglycerin(P < 0.05).The change rate of brachial artery diameter and body mass index, neck circumference, triglyceride, diastolic, systolic blood pressure and age were negatively correlated.Analyzing the results of AHI and vascular endothelial cell function between the two groups, the results showed that there was significant difference(p < 0.05).Meanwhile,the correlation between endothelial cell function and AHI was studied. The results showed that AHI was associated with endothelial function and the higher the AHI,the lower the values of FMD and NID.ConclusionsPatients with obstructive sleep apnea hypopnea syndrome can cause vascular endothelial dysfunction that are associated with body mass index, neck circumference, triglyceride, diastolic, systolic blood pressure and age. With the increase of AHI index, the degree of vascular endothelial function damage was more serious. |
