| ObjectiveThrough the construction of the blood purification nurse training curriculum system,this study provides a theoretical basis for the standardization,standardization and homogeneity training of the qualification access of new nurses in the blood purification center.Methods1.Literature Review: The research group used "blood purification /hematodialysis/hemodialysis,post competency/competency,Nurse training,training courses,Delphi method/Delphi" as Chinese retrieval words,using "blood Purification /hematodialysis/Hemodialysis,Competency,Nurse training,Training course,Delphi" as English Search terms.Search Cochrane library,Pubmed,Web of Science,EMbase,Ovid,Chinese Biomedical literature Database,CNKI,Wanfang Medical,VIP and other authoritative databases at home and abroad,analyze the collected literature,and preliminarily formulate the training course framework according to the results,Define core concepts such as post competency and curriculum system,and preliminarily form the framework and content of blood purification nurse training curriculum system.2.The semi-structured interview: the team selected 10 experts conducted semi-structured interview,including two blood purification nursing managers,two blood purification four blood purification specialist nurses,nursing educators,2 low qualification nurses,according to the expert advice and requirements to our research,enriching the content of this research training courses,and finalization of experts Zi Xun Biao.3.Delphi Letter Consultation: The research team selected 30 Delphi letter consultation experts from across the country in strict accordance with the expert inclusion criteria.Experts gave suggestions for the modification of each indicator in the questionnaire and assigned importance values.The research team modified the questionnaire according to the expert opinions in the two rounds of letter consultation,and finally the training course system of blood purification nurses based on post competency was determined.4.Statistical methods: Excel 2010 and SPSS 24.0 were used to calculate the mean,standard deviation and coefficient of variation of indicators at all levels,as well as the positive coefficient,authority coefficient and coordination coefficient of experts.Criteria for adding and deleting indicators: experts suggested adding or deleting indicators,and the research group discussed and agreed.In addition,if an index both meets the mean value of importance assignment ≤3.5 and coefficient of variation >0.25,it will be deleted.The proportion distribution method is used to calculate the weight.Results1.In the end,28 experts from 13 provinces and cities including Guangdong,Zhejiang,Sichuan and Shandong completed two rounds of letter inquiries.The positive coefficient of experts was 100% in the first round and 93.3% in the second round.The expert authority coefficient was 0.91.Kendall coordination coefficients are 0.255（P< 0.01）and 0.227（p< 0.01）.2.Two rounds of expert consultation form were issued in this study.The contents of the training system were modified according to the opinions of experts in the first round of letter consultation,and the second round of expert consultation form was prepared.The contents of the training system were further modified according to the opinions of experts in the second round of letter consultation,and the training course system of qualification access for blood purification nurses based on post competency was finally determined,which was composed of 3 first-level indicators,24 second-level indicators and 128 third-level indicators.3.In this study,the proportion distribution method is used to calculate the weight values of indicators at all levels and rank the importance of indicators.The weight values of the first-level indicators are ranked as follows: professional knowledge（0.3333）,professional skills（0.3358）and professional attitude（0.3308）.ConclusionBased on the post competency theory,this research is conducted through group discussion method,literature analysis method,semi-structured interview method and Delphi expert consultation method.Finally,the training content,training methods,training hours and other aspects composed of 3 first-level indicators,24 second-level indicators and 128 third-level indicators were determined,and the training course system based on post competency for blood purification nurses qualification access was established.The selected experts have high enthusiasm,high authority,regional representativeness,good consistency and coordination of expert opinions,and conform to the competency characteristics of blood purification nurses.This study is set up from several aspects,and finally builds the training course system of blood purification nurses’ qualification access,providing theoretical basis for the standardized,standardized and homogenized specialized training of new blood purification nurses.The next step is to apply this study to practice,observe the clinical effect,and continuously improve the training course system of blood purification nurse qualification access.
