Objective:To confirm the significance of circular RNA-cGmcl1 on the diagnosis,prognosis and Traditional Chinese Medicine(TCM)syndrome differentiation of heart failure by testing the circular RNA-cGmcl1 expressions in plasma of patients with heart failure and normal control group people,observing plasma cGmcl1 changes in different TCM syndromes and cardiac function classification of heart failure.Methods:35 patients with acute heart failure and 25 control subjects were enrolled from the department of emergency of Beijing Haidian hospital(Beijing haidian section of Peking University third hospital)from July 2015 to March 2016.The patients' basic information,symptoms,signs,tongue,pulse conditions,laboratory examinationsand echocardiographys data were collected and analyzed.Quantitative real-time PCR(qPCR)is applied to study the plasma cGmcl1 expression levels.The TCM syndrome type was differentiated according to the symptoms,signs,tongue and pulse conditions.Results:This research enrolled totally 35 heart failure patients,among which 8 cases were in NYHA functional ?,12 cases were in NYHA functional ?,and 15 cases were in NYHA functional?.Yang deficiency water diffusion syndrome(10 cases)and lung phlegm retention resistance syndrome(21 cases)were compared accordingto the TCM syndrome.Data analysis showed that the gender ratio,height,weight in control and heart failure groups,different heart function classification groups as well as in different TCM syndrome type groups showed no statistical significance.Additionally,the plasma cGmcl1 expression in heart failure group(0.722±0.743)significantly higher than that in normal control group(0.354±0.213),and the difference was statistically significant(p=0.019<0.05).With the increase of the cardiac function level,cGmcl1 expression also increased,which was a positive correlation between them,and there was significant statistical significance(p<0.01).The plasma cGmcl1 level in Yang deficiency water diffusion syndrome group was higher than that in lung phlegm retention resistance syndrome group,but results have no statistical significance(p=0.217>0.05).Plasma cGmcl1 expression level was negatively correlated with LVEF(r=-0.138,p=0.430),but positively related with the NT-proBNP(r=0.149,p=0.393).Area under ROC curve test analysis showed that the curve is 0.703,indicating that cGmcl1 could not only be used to diagnose and assess severity level of heart failure,but be a new index as NT-proBNP and LVEF to diagnose and evaluate heart failure.Conclusion:1.Plasma circular RNA—cGmcl1 may be as a new biomarker for heart failure;2.The expression level of plasma cGmcl1 is negatively related to the LVEF,positively related to the NT-proBNP,and the expression level of plasma cGmcl1 increases with heart failure severity.Plasma cGmcl1 can be used as a new marker of heart failure;3.The syndrome differentiation of TCM showed that plasma cGmcl1 expression in Yang deficiency water diffusion syndrome group was slightly higher than that in lung phlegm retention resistance syndrome group.
