Background Premature Coronary Artery Disease(pCAD)was defined as a cardiac event before the age of 65 for females.Compared with male patients of the same age,female patients tend to be more serious and have worse prognosis.At present,the pathogenesis of female premature coronary heart disease is not clear.On the other hand,in recent years,increasing evidence has suggested that sleep duration is an independent risk factor for coronary heart disease.Sleep duration that is either too short or too long affects the severity and prognosis of coronary heart disease.Female patients are more likely to have sleep disorders because of emotional or obesity problems.Therefore,to explore the relationship between sleep duration and severity of coronary heart disease in pCAD patients is of great significance for understanding the pathogenesis of pCAD and carrying out targeted prevention.Objective To explore the relationship between sleep duration and severity of coronary heart disease in female pCAD patients.Methods In this study we recruited female participants diagnosed pCAD by coronary angiography from October 2013 to March 2014 in Department of Cardiology,Fuwai Hospital,Chinese Academy of Medical Sciences.General characters?sleep conditions?coronary angiography findings and other clinical data were collected and analyzed.Three groups were defined according to sleep duration from Pittsburgh sleep quality index:short sleep duration<6h,reference sleep duration 6 to 8h,and long sleep duration?8h.Gensini score and the presence of acute coronary syndrome(ACS)were performed to quantify the severity of coronary heart disease.Through a cross-sectional study,we analyzed the relationship between sleep duration and the severity of coronary heart disease and further adjustment for known risk factors including hypertention,diabetes,hyperlipidemia and obesity to analyze whether sleep duration is an independent risk factor for the severity of coronary heart disease.Results 190 subjects were enrolled in the study,of which 61 were in the short sleep duration group,91 in the reference sleep duration group and 38 in the long sleep duration group.Compared to the reference sleep duration,short sleep duration is associated with a higher risk of ACS(OR 2.256,95%CI(1.082,4.705)P 0.030)and higher Gensini score(29.5±21.3 vs 23.1±15.5,P value<0.05).No differences were observed in long sleep duration group(P value>0.05).When adjusted for conventional risk factors,short sleep duration is still associated with higher Gensini_Score(P value<0.05).When adjusted for hypertension,no difference in the risk of ACS were observed between short sleep duration and reference sleep duration groups(P value>0.05).Conclusions Short sleep duration(<6h)is associated with a higher risk of ACS and higher Gensini score,and it is an independent risk factor for higher Gensini score.Sleep duration was not independent risk factor for the risk of ACS.Sleep duration may influence the risk of ACS by increasing the incidence of hypertention.There is no significant relationship between the long sleep duration(?8h)and the severity of coronary heart disease.
