ObjectiveTo observe the effect of navel application combined with salt pack hot ironing to prevent gastrointestinal reactions caused by intravenous injection of azithromycin in children.MethodsFrom February 2019 to June 2020,180 cases of children with azithromycin intravenous drip in a Third-Level First Class hospital in Changsha City were selected,and 6 cases were removed or dropped out in the research process.174 cases were included in this study.They were randomly divided into control group,umbilical compress group and umbilical compress combined with salt ironing group,58 cases in each group.The control group was given routine nursing(diet nursing,adjusting dropping speed,controlling concentration and observing the condition).The umbilical application group was given acupoint application of traditional Chinese medicine on Shenque acupoint on the basis of routine nursing.The umbilical application combined with salt ironing group was given acupoint application of traditional Chinese medicine on Shenque on the basis of routine nursing,and the Shenque acupoint was hot ironed with coarse salt.Collect the basic information of children,observe and record the occurrence of gastrointestinal reaction and symptom score during intravenous azithromycin infusion.The higher the score,the more serious the gastrointestinal reaction.The data were analyzed by SPSS23.0 sofware.Measurement data are expressed as(?)± s.One way ANOVA was used to compare normal distribution data between groups,and Kruskal Wallis test was used to compare non normal distribution data between groups.Qualitative data were described by percentage,and chi square test was used for comparison between groups.The rank data were described by percentage,and the comparison between groups was performed by multiple rank sum test.The difference was statistically significant(P<0.05).The incidence of gastrointestinal reactions in children of different gender and age were compared.The incidence,degree,form and treatment compliance of gastrointestinal reactions in three groups were compared.Results(1)Comparison of gender and age of gastrointestinal response of azithromycin in children.There was no significant difference in gender in the gastrointestinal response of children with azithromycin(P>0.05).The incidence of gastrointestinal reactions was closely related to age.The incidence of gastrointestinal reactions was 29.8%in children aged 3-6,17.7%in children aged 7-12 and 0%in children aged 13-14.The lower the age,the higher the incidence of gastrointestinal reactions(P<0.05).(2)Comparison of the incidence of gastrointestinal reactions among the three groups.The incidence of gastrointestinal reaction in the umbilical application combined with salt ironing group was6.9%,which was significantly lower than that in control group(39.7%)and the umbilical application group(25.9%).There was significant difference among the three groups(P<0.05).(3)Three groups of children with gastrointestinal reaction degree comparison.The gastrointestinal reaction score of the umbilical application combined with salt ironing group was1.50±0.58,which was lower than that of control group(7.26±1.71)and the umbilical application group(4.53 ± 1.12),and the difference was statistically significant(P<0.05).(4)The incidences of nausea,vomiting and diarrhea in the umbilical application group were 12.1%,10.3%and 5.2%,which were lower than those in the control group(31.0%,22.4%and 12.1%).The score of nausea,vomiting and diarrhea in the umbilical application group was 2.38±0.92?2.00±1.00,which were lower than in the control group 4.11±0.81?4.00±0.58.The difference between the two groups was statistically significant(P<0.05).while the incidence of abdominal pain(19.0%)and symptom score of abdominal pain(3.91±0.83),compared with the control group(4.69±0.63),there was no significant difference between the two groups(P>0.05).The incidences of nausea,vomiting,abdominal pain and diarrhea in the umbilical application combined with salt ironing group(1.7%,1.7%,3.4%and 0.0%)were lower than those in the control group(31.0%,22.4%,22.4%and 12.1%),and those in the umbilical application(12.1%,10.3%,19.0%and 5.2%).The symptom scores of nausea,vomiting,abdominal pain and diarrhea in the umbilical application combined with salt ironing(1.50±0.71?1.50±0.71?0.00±0.00)were lower than those in the control group(4.11 ±0.81?4.69±0.63?4.00±0.58)and umbilical application group(2.38±0.92?3.91 ±0.83?2.00±1.00),the difference was statistically significant(P<0.05).(5)Comparison of treatment compliance:the complete compliance rate of the umbilical application combined with salt ironing was 93.1%,which was higher than that of control group(63.9%)and umbilical application group(79.3%).There was significant difference among the three groups(P<0.05).Conclusion(1)The lower the age of children,the higher the incidence of gastrointestinal reactions.(2)Umbilical application combined with hot pressing in salt package can reduce the incidence and severity of gastrointestinal reactions caused by azithromycin,and the effect is better than umbilical application with Chinese medicine alone.(3)Umbilical Compress combined with salt pack hot ironing can reduce the incidence and severity of gastrointestinal symptoms such as nausea and vomiting,abdominal pain and diarrhea,and the effect is more obvious in reducing the incidence and severity of abdominal pain compared with Umbilical Compress with traditional Chinese medicine.(4)Umbilical Compress combined with hot ironing can improve the treatment compliance of children.
