Clinical Observation On Acupoint Application Of Jiawei Fuzi Dingxiang Powder In The Treatment Of Gastroparesis Syndrome (spleen And Stomach Deficiency-cold Syndrome) After Gastrointestinal Tumor Operation

Objective: to observe the clinical effect of aconite clove powder acupoint application in the treatment of spleen and stomach deficiency cold type of gastric paralysis syndrome after operation of digestive tract tumor,and to explore the clinical value of this prescription in the treatment of gastroparesis syndrome after operation of digestive tract tumor.To provide a new way of thinking and method for the treatment of gastroparesis syndrome after operation of digestive tract tumor.Methods: A total of 30 patients with gastric paralysis syndrome with spleen and stomach deficiency cold type who were admitted to the Department of Hepatobiliary,pancreatic,spleen and gastrointestinal surgery and the Department of anorectal surgery in the First Hospital of Traditional Chinese Medicine of Changde City,Hunan Province from May 2020 to May 2021 were randomly divided into control group and treatment group,with 15 patients in each group.The control group was treated with basic western medicine,and the treatment group was additionally treated with acupoint application of aconite clove powder for external use.7 days as a course of treatment,the two groups were treated for 14 consecutive days;Evaluation indicators:(1)Changes of gastric tube drainage(ml)before treatment,day 1,day 3,day 7 and day 14;(2)Changes of TCM symptom score before treatment,day 7 and day 14;(3)The changes of serum gastrin in 2 groups before treatment and on the 7th and 14 th day after treatment;(4)Treatment effect of the two groups: the change of total effective rate(total effective rate = significant efficiency +recovery rate)after 7 days and 14 days of treatment.(5)The time of recovery of gastrointestinal peristalsis,hospital stay,hospital expenses and complications after operation in both groups.Results :(1)There was no significant difference in gastric tube drainage(ml)between the 2 groups before treatment(P>0.05),and the gastric tube drainage flow decreased with time on day 1,day 3,day7,day 14 after treatment(P<0.05).There was no significant difference between the 2 groups before treatment,day 1 and day 3after treatment(P>0.05),and between the 7 and 14 days after treatment(P<0.05)and the decrease of gastric tube drainage in treatment group was higher than that in control group(P<0.05).(2)There was no significant difference in TCM syndrome score between the two groups before treatment(P>0.05).The TCM symptom score of the two groups decreased with time on the 7th and 14 th day after treatment,which was statistically significant(P<0.05)compared with the TCM symptom score before treatment,and the decline of the treatment group was higher than that of the control group,which has statistical differences(P<0.05).(3)There was no significant difference in serum gastrin between the two groups before treatment(P>0.05),but it showed a downward trend with time on the 7th and14 th day after treatment,and the difference was statistically significant(P<0.05)compared with that before treatment,and the decrease in the treatment group was higher than that in the control group,with statistical difference(P<0.05)(4)The total effective rate on day 7 and day 14 in group 2 was on the rise.The total effective rate of the treatment group was higher than that of the control group(P<0.05)(5)The time of gastric tube removal was 10.33±3.04 d in the treatment group.Hospital stay was 17.33±3.44 d.Hospital expenses were 2.41±6300 yuan.Time of gastric tube removal was 14.87±3.23 d in control group.Hospital stay was 22.33±3.05 d.Hospital expenses were 3.67±8600 yuan;No complications occurred in either group.After operation,gastric tube extubation time,hospitalization time and hospitalization cost were statistically significant(P<0.05).The treatment group was significantly better than the control group.Conclusion:aconite clove powder acupoint application can obviously improve the symptoms of abdominal pain,abdominal distension,vomiting and nausea in the cold type of spleen and stomach of gastrointestinal paralysis syndrome after operation of digestive tract tumor.It accelerates the recovery of gastrointestinal function time,and improves the quality of life of patients.It reduces the hospitalization time and hospitalization cost of patients,thus improve the clinical curative effect of treatment for PGS,and proves its safety and feasibility.It provides a safe,effective,simple,economical and effective treatment method of integrated Chinese and western medicine for clinic.
