| Objective:By observing the clinical efficacy of external application of Sanhuang ointment after calcaneal fracture,studying its mechanism in reducing swelling and relieving pain,discussing the significance of stasis-heat syndrome in the occurrence and development of pain and swelling of comminuted calcaneal fracture,and promoting the advantages of external application of traditional Chinese medicine in the treatment of fracture pain and swelling.Methods:Methods:42 cases of calcaneal comminuted fracture were numbered according to the admission time,and then randomly divided into experimental group and control group by random number table method.There were 22 cases in the experimental group,2 cases of falling off,20 cases in the final,and 20 cases in the control group.The control group was given 200 ml mannitol injection by intravenous drip,once a day,continuous treatment for 7 days,braking rest,raising the affected limb for 30 minutes°、The patients in the experimental group were additionally treated with Sanhuang Ointment on the heel on the basis of the control group.Both groups were treated for one week.The analgesic effect was evaluated according to the change of pain score,the swelling reducing effect was evaluated according to the difference of heel ankle circumference and the time of the first dermatoglyphic appearance;The VAS score,the difference of heel ankle circumference,the time of first dermatoglyphic appearance and TCM syndrome score was used to evaluate the effectiveness of the two groups.Results:(1)In this study,42 patients were included in the treatment period,a total of 2 cases fell off,the actual 40 patients completed the treatment;(2)There were no significant differences in gender,age,circumference of contralateral side between the two groups(P>0.05);(3)The VAS scores of the two groups on the 7th day and the 1st day after admission were compared(P<0.05),which showed that the pain of the two groups were alleviated after treatment,but there was no significant difference in the improvement of pain;(4)The difference of ankle circumference between the two groups was statistically significant on the 5th and 7th day of treatment(P<0.05),and the trend of ankle circumference difference between the affected side and the healthy side of the two groups before and after treatment showed a trend of first increase and then decrease,but the change trend of the two groups was not completely consistent,indicating that the two groups had the effect of improving swelling on the 5th and 7th day of admission,The swelling of heel and ankle began to decrease after reaching the highest value;(5)The time of the first observation of skin veins in the experimental group was earlier than that in the control group(P<0.05),indicating that the detumescence efficiency of the experimental group was faster;(6)Two groups of TCM syndrome integral curative effect judgment,the experimental group cured 0 cases,markedly effective in 12 cases,effective in 8 cases,invalid in 0 cases,the total effective rate of 100%,the control group cured 0 cases,markedly effective in 4 cases,effective in 13 cases,invalid in 3 cases,the total effective rate of 85%,the difference between the groups was statistically significant(P<0.05),indicating that the experimental group cooling blood stasis effect is more obvious.Conclusion:External application of Sanhuang Ointment can reduce the pain of patients with comminuted calcaneal fracture and improve local swelling.The important thing is that it has simple operation,low price and small side effects.It can directly act on the affected area and the operation is simple.It can also avoid the bitter taste of patients when they take oral Chinese medicine.The patient’s satisfaction is high and it is worthy of promotion in clinical practice... |
