ObjectiveTo research the difference between laparoscopic and open portal devascularization in the treatment of portal hypertension.MethodsFrom October 2019 to December 2020,patients with cirrhotic portal hypertension treated in Taizhou and Subei hospital were selected as subjects.A total of 80 patients were collected in this study,and 8 patients with poor liver function were excluded.Using a random number table,the patients were divided into control group and research group,with 40 patients in each group.The research group was treated with laparoscopic portal-azygous disconnection,while the control group with laparotomy combined with portal-azygous disconnection.The intraoperative blood loss,postoperative time of anal exhaust,surgical duration,postoperative total drainage volume,changes in platelet count and the incidence of complications were compared between the two groups.After collecting these data,statistical analysis was carried out using SPSS 20.0.The measurement data were expressed as((?) ± s)and analyzed by the t test.The enumeration data were analyzed using the ? 2 test.P < 0.05 was considered as statistically significant.ResultsIt was found that the postoperative drainage volume and intraoperative blood loss in the study group were significantly less than those in the control group.In addition,the time of getting out of bed and anal exhaust in the study group was shorter than that in the control group,and the difference between the two groups was statistically significant(P< 0.05).The total cost of hospitalization in the control group was lower than that in the study group.The comparison of postoperative data between the two groups showed that the proportion of postoperative complications in the control group was much higher than that in the study group,including fever,hepatic ascites,portal vein thrombosis,pleural effusion,etc.,and the difference between the two groups was statistically significant(P < 0.05).There was no difference in length of stay between the two groups.There was no significant difference in the number of platelets between the two groups at 1d,3d,7d and 14 d postoperatively(P > 0.05),but the number of platelets in both groups showed an upward trend in the two weeks postoperatively.ConclusionLaparoscopic portal-azygous disconnection in the treatment of patients with cirrhotic portal hypertension has many advantages,such as few postoperative complications,little intraoperative blood loss,short surgical duration and rapid postoperative recovery,but the hospitalization expenses are higher.By comparing the advantages and disadvantages of laparotomy and laparoscopy combined with portal-azygous disconnection,laparoscopic portal-azygous disconnection is worthy of clinical promotion and application.
