Clinical Observation Of Mankuangmengxing Decoction In The Treatment Of Postoperative Delirium In Elderly Patients With Lower Extremity Fractures

Purpose:By observing,analyzing and evaluating the curative effect of Diankuang Meng xing Decoction Modified Recipe in the treatment of postoperative delirium in the elderly with lower extremity fractures,it is expected to provide new ideas and methods for the treatment of postoperative delirium in the elderly with lower extremity fractures.Material and method: From August 2018 to October 2020,postoperative delirium in the hospitalized elderly with lower extremity fractures in the Department 1 of Orthopedics and Traumatology,Affiliated Hospital of Liaoning University of Traditional Chinese Medicine were collected.62 patients who met the inclusion criteria of this observation but did not meet the exclusion criteria were selected and the random numbers table method was used.62 patients were randomly divided into olanzapine group and TCM group,31 cases in each group.Collected the patient’s age,gender,education level,operation style,operation time,blood loss and other general information.The olanzapine group was treated with olanzapine alone,while the TCM group was treated with Diankuang Mengxing Decoction Modified Recipe on the basis of olanzapine.Delirium Rating Scale(DRS),Brief Psychiatric Rating Scale(BPRS),Clinical Global Impression Scale(CGI)and TCM symptom score were used to evaluate the efficacy.The efficacy was observed for 2 week.Finally,the overall efficacy was evaluated.Spss25.0 statistical software was used for statistical description and correlation analysis.Results:1.Comparison of general information between the TCM group and olanzapine group: there was no significant difference in general information(age,gender,education level,operation mode,operation time,anaesthesia time,blood loss)between the two groups(P>0.05),which was comparable;2.Compared with the self before treatment,the CGI score in the two groups decreased significantly after two weeks of treatment(P<0.05)and there was no significant difference between the two groups(P>0.05);3.Compared with the self before treatment,the DRS score of TCM group and olanzapine group decreased significantly after two weeks of treatment(P<0.05),and there was no significant difference between the two groups(P>0.05);4.Compared with the self before treatment,the TCM symptom score of delirium in the two groups decreased significantly after two weeks of treatment,the difference was statistically significant(P<0.05)and there was no significant difference between the two groups(P>0.05);5.In the comparison of clinical curative effect,there was no statistical significance between the traditional Chinese medicine group and olanzapine group(P>0.05);6.Compared with olanzapine group,the duration of delirium and hospital stay in TCM group were significantly lower(P<0.05);7.In the safety comparison,the incidence of adverse reactions between the two groups was not statistically significant(P>0.05).Conclusion:Diankuang Mengxing Decoction Modified Recipe combined with olanzapine has good curative effect on postoperative delirium in elderly patients with lower limb fracture,and the curative effect is exact.It can shorten the duration of postoperative delirium,help patients to carry out early rehabilitation exercise,which is worthy of clinical promotion.
