Purpose : The purpose of this study is to explore the distribution of TCM syndrome characteristics of patients with Type 2 Diabetic Nephropathy(DN)in Singapore, and to observe the nature and characteristics of its occurrence and development to determine the main clinical manifestations of each syndrome type and formulate disease-syndrome.The research model of symptom-treatment provides a standardized basis for TCM clinical syndrome differentiation, which is expected to enhance the foundation of Singapore TCM treatment of diabetic nephropathy.Material and Method: Collect data from 80 patients with diabetic nephropathy in the outpatient department of the Diabetes Department of Chung Hwa Hospital from December2020 to July 2021 for data collection and questionnaire surveys, including general data, laboratory indicators, health check reports and other medical conditions and TCM syndromes.Syndrome classification and measurement table, descriptive analysis of TCM syndromes of patients based on the results and judgment criteria. After the database is established through Excel, the SPSS22.0 system will be used to statistically analyze the TCM syndrome characteristics and distribution status of Type 2 Diabetic Nephropathy Stage ? in Singapore.Results:1 The ten most frequent symptoms of the main clinical manifestations of Singapore patients with Stage ? Diabetic Nephropathy in this study are: fatigue(97.5%),backache and weak knees(95.0%),shortness of breath and weak to speak(85.0%),dry throat and mouth(67.5%),edema of limbs(60.0%),bloated stomach(56.25%),turbid urine(52.5%),poor appetite(50.0%),frequent urination(50.0%),dark purple lips(47.5%).2 According to the observation of the color of the tongue,Singapore patients with Stage ?Diabetic Nephropathy is mainly dark red in color with petechiae and ecchymosis. The appearance of the tongue that showed dark purple in color(30.0%),followed by flabby tongue(22.5%),indicating that blood stasis and Qi deficiency run through the entire course of the disease. The most frequent occurrence of tongue coating is white and slimy(37.5%),followed by yellow and slimy coating(22.5%)and white thick coating(18.75%).Thick slimy tongue coating indicates that patients with Stage ? Diabetic Nephropathy suffered from water retention, heat and phlegm stagnation in their bodies.3 According to the observation of pulse conditions, the pulse condition of Singapore type 2diabetic nephropathy stage III patients is mainly composed of low thready pulse(36.25%),followed by rapid thready pulse(22.5%),flowing-like pulse(15.0%)and tense-bow pulse(13.75%)4 From the collected clinical symptoms of Singapore patients with type 2 diabetic nephropathy stage III under TCM syndrome differentiation, the results showed that Qi and Yin deficiency is 35%,Spleen and Kidney Qi deficiency is 31.25%,followed by Liver and Kidney Yin deficiency Syndrome accounted for 15%,Yin deficiency with dry heat syndrome accounted for 12.5%,and Spleen and Kidney Yang deficiency syndrome accounted for 6.3%.5 Based on the collected clinical symptoms of Singapore patients with type 2 diabetic nephropathy stage III under TCM syndrome differentiation, the standard empirical results are blood stasis syndrome accounted for 45%,damp-heat syndrome accounted for 27.5%,damp-turbid syndrome accounted for 21.25%,and water qi syndrome accounted for 21.25%.6.25%.6 Observation in terms of gender, there is no statistically significant difference in the distribution of symptoms of various TCM syndromes,P>0.05.7 The average body mass index(BMI)calculated from collected weight and height data is27.2.Singapore type 2 diabetic nephropathy stage III patients are all overweight.8 In this study, the total number of patients over 40 years old with Stage ? Diabetic Nephropathy amounted to 95%,and the majority of patients were between 51-60 years old, amounting to 41.3%.9 In the observation of the excretion rate of continuous microalbuminuria,Singapore type 2diabetic nephropathy patients with different TCM syndromes found that the excretion rate of continuous microalbuminuria of 201-300 mg/g is most common with the deficiency of Spleen and Kidney Qi which accounted for 25.0%.The excretion rate of persistent microalbuminuria at 101-200mg/g is mostly due to deficiency of both Qi and Yin,accounting for 15.0%,while the excretion rate for persistent microalbuminuria at 30-100mg/g is Qi Yin deficiency syndrome, accounting for 7.5%.Conclusion:1 The 10 main symptoms of Singapore patients with Type 2 Diabetic Nephropathy Stage ?are fatigue, backache and weak knees, shortness of breath and weak to speak, dry throat and mouth, edema of limbs, bloated stomach, turbid urine, poor appetite, frequent urination and dark purple lips.2 The main tongue appearance of Singapore patients with Type 2 Diabetic Nephropathy Stage? is dark purple tongue and slimy tongue coating.3 The main pulse condition of Singapore patients with Type 2 Diabetic Nephropathy Stage ?is low thready pulse.4 The TCM diagnosed with main weakness in physics for Singapore patients with Type 2Diabetic Nephropathy Stage ? are mainly Qi combined with Yin deficiency(based on symptoms of fatigue, dry throat and mouth, weak to speak, flabby tongue and thready pulse etc)and Spleen Qi combined with Kidney Qi deficiency(based on symptoms of fatigue, weak to speak, poor appetite, bloated stomach, backache and weak knees, edema of limbs, turbid urine, frequent urination, flabby tongue, and low thready pulse etc).5 The TCM diagnosed with other solid conditions in physics for Singapore patients with Type2 Diabetic Nephropathy Stage ? is blood stasis.
